Elintavoilla, kuten ravitsemuksella voidaan vaikuttaa sairastumisriskiin, mutta tutkimusta on viel\u00e4 rajallisesti saatavilla. T\u00e4m\u00e4n pro gradu -tutkielman tarkoitus oli selvitt\u00e4\u00e4 Lynch syndrooma -geenikantajien energia- ja suojaravintoaineiden saantia ja onko naisilla ja miehill\u00e4 eroa saannissa. Lis\u00e4ksi tarkasteltiin, miten yli 60-vuotiaiden Lynch syndrooma -geenikantajien energia- ja suojaravintoaineiden saanti eroaa kolorektaalisy\u00f6p\u00e4\u00e4n sairastuneiden ja sairastumattomien v\u00e4lill\u00e4. \n\nTutkielmassa k\u00e4ytettiin vuosina 2016\u20132020 Lynch syndrooma ja el\u00e4m\u00e4ntavat -tutkimusta varten ker\u00e4tty\u00e4 ruoank\u00e4ytt\u00f6kyselyn poikkileikkausaineistoa. Tutkittavat (n = 458) saatiin Suomen Lynch syndrooma -rekisterist\u00e4. Vastaajien energia- ja suojaravintoaineiden saantia ravinnosta tarkasteltiin frekvenssein ja prosenttiosuuksin ja tuloksia verrattiin suomalaisiin ravintosuosituksiin. Naisten ja miesten eroja energia- ja suojaravintoaineiden keskim\u00e4\u00e4r\u00e4isess\u00e4 saannissa tutkittiin t-testill\u00e4 ja Mann Whitney -U-testill\u00e4. Yli 60-vuotiaiden kolorektaalisy\u00f6p\u00e4\u00e4n sairastuneiden ja sairastumattomien energia- ja suojaravintoaineiden saantia arvioitiin ristiintaulukoimalla, \u03c72-testill\u00e4 ja Fisherin tarkalla testill\u00e4 ja yhteytt\u00e4 kolorektaalisy\u00f6p\u00e4\u00e4n analysoitiin logistisen regressioanalyysin avulla. \n\nVastaajista saantisuositusten mukaan sai kuitua 35,2 %, proteiinia 15,6 %, rasvoja 71,4 % ja D-vitamiinia 69,9 % ja kalsiumia 81,2 %. Miehet saivat naisia keskim\u00e4\u00e4r\u00e4ist\u00e4 enemm\u00e4n D-vitamiinia (13,0 %), kalsiumia (8,7 %) ja proteiinia (7,1 %), ero oli tilastollisesti merkitsev\u00e4. Energia- ja suojaravintoaineiden saannissa ei ollut tilastollisesti merkitsev\u00e4\u00e4 eroa yli 60 vuotta t\u00e4ytt\u00e4neiden (n = 223) kolorektaalisy\u00f6p\u00e4\u00e4n sairastuneiden ja sairastumattomien v\u00e4lill\u00e4. Bin\u00e4\u00e4rinen logistinen regressioanalyysi osoitti, ett\u00e4 kuidun, proteiinin, rasvan, D-vitamiinin ja kalsiumin saanti ei ollut yhteydess\u00e4 kolorektaalisy\u00f6p\u00e4\u00e4n, kun mallit oli vakioitu i\u00e4ll\u00e4, sukupuolella, tupakoinnilla ja koulutustasolla. \n\nSuurin osa Lynch syndrooma -geenikantajista sai ravinnosta kuituja liian v\u00e4h\u00e4n ja proteiinia liikaa. Rasvan saanti oli suositusten yl\u00e4rajoilla. D-vitamiinin ja kalsiumin saanti oli useimmilla suositusten mukaista. Vaikka yhteytt\u00e4 energia- ja suojaravintoaineiden ja kolorektaalisy\u00f6v\u00e4n v\u00e4lill\u00e4 ei l\u00f6ytynyt, on mahdollista, ett\u00e4 tutkittavien tiedossa oleva sy\u00f6p\u00e4riski ja/tai sairastettu sy\u00f6p\u00e4 ja ravitsemusneuvonta on tasoittanut eroja poikkileikkausaineiston ryhmien v\u00e4lill\u00e4. Jatkossa tarvitaan lis\u00e4\u00e4 tutkimusta naisten ja miesten energia- ja suojaravintoaineiden saannin koostumuksesta ja eroista niiden v\u00e4lill\u00e4, jotta ravitsemusta voidaan ohjata paremmin terveytt\u00e4 tukevaksi.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "Lynch syndrome is a hereditary genetic condition predisposing to disorder in DNA repair system and increases especially the risk of colorectal cancer (colon and rectum) in particular. Lifestyle habits such as nutrition can affect the risk of contracting the illness, but research is still limited. The purpose of this master\u2019s thesis was to investigate the intake of energy and protective nutrients by Lynch syndrome carriers and whether there is a difference in intake between women and men. It also examined how the energy and protective nutrient intake of Lynch syndrome carriers over 60 years of age differed between those with and without colorectal cancer. \n\nThe thesis incorporated cross-sectional data from the food frequency questionnaire (FFQ) survey collected for the 2016\u20132020 Lynch syndrome and lifestyle study. Subjects (n = 458) with a genetic disorder predisposing to cancer were obtained from the Finnish Lynch syndrome registry. Respondents\u2019 intakes of energy and protective nutrients from food were examined in terms of frequencies and percentages, and the results were compared with the Finnish nutritional recommendations. Differences in mean energy and protective nutrient intake between women and men were examined using the t-test and the Mann Whitney-U test. Energy and protective nutrient intake of colorectal cancer patients over 60 years of age were assessed by cross-tabulation, \u03c72 test, and Fisher\u2019s precise test, and the association with colorectal cancer was analysed by logistic regression analysis. \n\nAccording to nutritional recommendations, respondents received 35.2% fibre, 15.6% protein, 71.4% fats, 69.9% D- vitamin and 81.2 % calcium. On average, men received more vitamin-D (13.0%), calcium (8.7%), and protein (7.1%) than women; the difference was statistically significant. There was no statistically significant difference in energy and protective nutrient intake between those with colorectal cancer and those over 60 years of age (n = 223). Binary logistic regression analysis showed that fibre, protein, fat, vitamin D, and calcium intake were not associated with colorectal cancer when models were standardised by age, gender, smoking, and level of education. \n\nThe major proportion of Lynch syndrome carriers received from nutrition too little fibres and too much protein. Fat intake was above the upper limits of the recommendations. Intake of vitamin D and calcium was in accordance with most recommendations. Although no association was found between energy and protective nutrients and colorectal cancer, it is possible that the known cancer risk and/or cancer and nutritional counselling of the subjects offset the differences between the groups.
