Tarkastelemme myös, miten lapsi etääntymisaloitteillaan uudelleenasemoi itseään vuorovaikutukseen ja kuinka terapeuttisen systeemin aikuiset tukevat lapsen vuorovaikutustoimintaa. Perinteisen poliklinikkaympäristön lisäksi tutkimme näitä ilmiöitä kotona toteutetussa perheterapiaprosessissa, sillä koti voi terapian tavoitteiden toteutumisen kannalta tarjota turvallisen ja lasta osallistavan ympäristön. Aineistomme koostuu Kuopion Yliopistollisen sairaalan lastenpsykiatrian poliklinikalla ja kotona toteutetuista perheterapiaistuntojen ääni- ja videotallenteista. Analyysimenetelmänä käytimme keskustelunanalyysiä. Havaitsimme, että terapeuttisen systeemin aikuiset asemoivat lasta itseään ja ei itseään koskevan keskustelun sivustaseuraajaksi, selontekovelvolliseksi ja kuunteluvelvolliseksi. Lisäksi havaitsimme lapsen siirtyvän aktiivisesta keskustelijasta keskustelun passiiviseksi osapuoleksi. Tyypillisesti lapsi vastusti tai haastoi aikuisten hänelle määrittämää asemaa vuorovaikutuksessa tai vetäytyi siitä. Terapeuttien ja vanhempien responssit lapsen etääntymisaloitteisiin ilmenivät sekä toimimattomina että lapsen vuorovaikutusta tukevina. Tuloksemme osoittavat haasteen osallistaa lapsi tasavertaiseksi vuorovaikutukseen osallistujaksi. Lasten etääntymis- ja vuorovaikutusaloitteet olivat tyypillisesti epäkonstruktiivisia, minkä vuoksi heidän häiriökäyttäytymisensä taakse kätkeytyvä mielekäs tavoite kuulluksi ja ymmärretyksi tulemisesta voi jäädä huomiotta. Tutkimuksemme antoi myös viitteitä siitä, että lapsen etäisyyden säätely voi olla kontekstisidonnaista kodin mahdollisesti tarjotessa lapsen vuorovaikutustoiminnan kannalta osallistavamman toimintaympäristön. Kokonaisuudessaan tutkimus tarjoaa uutta tietoa lapsen asemoitumisesta perheterapiassa sekä lisää perheterapiaan liittyvien vuorovaikutusilmiöiden ymmärrystä. The core idea of family therapy is its` dialogue but based on previous research it is known that children have unequal position in family therapy interaction. In this study we have extended the perspective of previous studies. For the first time we investigate children`s positioning in family therapy in children with oppositional defiant disorder or conduct disorder. We also examine how child repositions himself or herself by using disengagement strategies and how the adults of the therapeutic system support the interaction of the child. In addition to the traditional policlinical environment, we investigate these phenomena also at home based family therapy process. It is known that home may offer safe and involving environment for children. Our data consisted of audio and video-taped family therapy sessions which were accomplished at home or at Kuopio University Hospital Child Psychiatry clinic. The data was analyzed by using conversational analysis. We noticed that adults of the therapeutic system positioned children as a side follower of conversations concerning children or general topics, account responsible and listening responsible. In addition, we perceived that children shift from active conversationalist to passive party of the conversation. Typically, children resisted or challenged the position offered by adult parties or disengaged from the interaction. The responses of therapists and parents to children's disengagement strategies were dysfunctional or supported children's interaction. Our results indicate that there is a challenge in involving children as an equal part of the interaction. The disengagement and interaction strategies of the children are typically deconstructive and therefore the reasonable message behind their disruptive behavior, to become understood and heard, may be ignored. Our results suggest that the regulation of disengagement may be context related. Home may be involving environment for children within family therapy context. Overall, our study provides new information on children`s positioning in family therapy and improves our current knowledge regarding the interaction phenomena in therapeutic practice.
