Nivelrikon ja luun lujuusominaisuuksien v\u00e4list\u00e4 yhteytt\u00e4 on tutkittu vuosikymmeni\u00e4 ilman selkeit\u00e4 johtop\u00e4\u00e4t\u00f6ksi\u00e4. Aiemmat tutkimukset ovat keskittyneet luun mineraalitiheyden (BMD) tutkimiseen, vaikka luun lujuuteen vaikuttavat my\u00f6s sen mineraalim\u00e4\u00e4r\u00e4 (BMC) sek\u00e4 geometriset ominaisuudet. T\u00e4m\u00e4n tutkielman tarkoituksena oli selvitt\u00e4\u00e4, onko reisiluun kaulan mineraalim\u00e4\u00e4r\u00e4 yhteydess\u00e4 nivelrikon radiologiseen luokitukseen liev\u00e4asteista polven nivelrikkoa sairastavilla postmenopausaalisilla naisilla.\nTutkielmassa hy\u00f6dynnettiin Jyv\u00e4skyl\u00e4n yliopiston \u201cLiikuntaa luulle ja nivelrustolle\u201d (LuRu2) -tutkimuksen alkumittausaineistoa. Tutkittavat (n = 103) olivat 55\u201375-vuotiaita postmenopausaalisia naisia, joilla oli todettu radiologisesti liev\u00e4 (K/L 1-2) polven nivelrikko. Tutkittavien BMC mitattiin molemmista reisiluun kauloista kaksienergisen r\u00f6ntgens\u00e4teen absorptiometrian (DXA) avulla. Polven nivelrikon radiologinen vaikeusaste m\u00e4\u00e4ritettiin r\u00f6ntgenkuvaukseen perustuvan Kellgren-Lawrence-luokituksen avulla. Taustamuuttujia olivat tutkittavien ik\u00e4, pituus, paino, painoindeksi sek\u00e4 fyysinen aktiivisuus (METmin/vko). Aineisto analysoitiin IBM SPSS Statistics 28-ohjelmalla. Reisiluun kaulan BMC:n ja polven nivelrikon radiologisen luokituksen yhteytt\u00e4 tarkasteltiin yksisuuntaisen varianssianalyysin (ANOVA) sek\u00e4 kovarianssianalyysin (ANCOVA) avulla. Reisiluun kaulan BMC:n ja taustamuuttujien v\u00e4lisi\u00e4 yhteyksi\u00e4 tarkasteltiin Pearsonin korrelaatiokertoimien avulla.\nTulokset osoittivat, ett\u00e4 reisiluun kaulan BMC kasvoi polven nivelrikon radiologisen luokituksen edetess\u00e4 oikean alaraajan osalta (p = 0,04), mutta vasemman alaraajan osalta tilastollisesti merkitsevi\u00e4 eroja ei havaittu. Oikeassa alaraajassa lievien nivelrikkomuutosten K/L 2-ryhm\u00e4ss\u00e4 BMC (4,47 g) oli 12,1 % suurempi kuin nivelrikon osalta terveiden polvien K/L 0-ryhm\u00e4ss\u00e4 (3,93 g) (p = 0,034). Kovarianssianalyysiss\u00e4 pituudella ja painolla korjattu malli osoitti, ettei nivelrikon luokitus selitt\u00e4nyt mineraalim\u00e4\u00e4r\u00e4n vaihtelua oikean (F = 1,73, p = 0,18, \u03b72 = 0,03) tai vasemman (F = 1,83, p = 0,17, \u03b72 = 0,04) alaraajan osalta.\nLiev\u00e4\u00e4 polven nivelrikkoa sairastavien postmenopausaalisten naisten radiologiset polven nivelrikkomuutokset ja reisiluun kaulan BMC eiv\u00e4t olleet yhteydess\u00e4 toisiinsa, kun sekoittavat tekij\u00e4t otettiin huomioon. Its prevalence has increased with population aging, alongside deteriorating bone health and rising rates of osteoporotic fractures. Risk factors for both knee OA and bone fragility include aging and female sex. The relationship between OA and bone strength characteristics has been studied for decades without clear conclusions. While previous research has focused primarily on bone mineral density (BMD), other critical indicators of bone strength, such as bone mineral content (BMC) and bone geometric properties, have received less attention. The aim of this thesis was to investigate whether femoral neck BMC is associated with the radiological classification of osteoarthritis in postmenopausal women with mild knee OA.\nThe study utilized baseline data from the \"Effects of Multicomponent Exercise on Subchondral Bone and Cartilage in Postmenopausal Women With Knee Osteoarthritis\" (LuRu2) study conducted at the University of Jyv\u00e4skyl\u00e4. Participants (n = 103) were 55\u201375-year-old postmenopausal women diagnosed with mild (K/L 1\u20132) radiographic knee OA. BMC was measured in both femoral necks using dual-energy X-ray absorptiometry (DXA). The radiographic severity of knee OA was classified using the Kellgren-Lawrence (K/L) grading system. Background variables included participants\u2019 age, height, weight, body mass index, and physical activity (MET-minutes/week). Data were analyzed using IBM SPSS Statistics 28. The association between femoral neck BMC and knee OA severity was examined using one-way ANOVA and ANCOVA. Pearson correlation coefficients were used to analyze the relationships between BMC and background variables.\nThe results showed that femoral neck BMC increased with advancing radiographic severity of knee OA in the right leg (p = 0,04), whereas no statistically significant differences were observed in the left leg. In the right leg, the BMC in the mild OA K/L 2 group (4,47 g) was 12,1% higher than in the K/L 0 group (3,93 g) (p = 0,034). However, the ANCOVA model adjusted for height and weight indicated that OA severity did not explain the variation in BMC for the right (F = 1,73, p = 0.18, \u03b7\u00b2 = 0,03) or left leg (F = 1,83, p = 0.17, \u03b7\u00b2 = 0,04).\nIn conclusion, radiographic knee OA changes in postmenopausal women with mild knee OA were not associated with femoral neck BMC after accounting for confounding factors. The observed increase in right femoral neck BMC in the K/L 2 group supports earlier evidence of enhanced bone strength characteristics in mild OA.
