Suomessa perusterveydenhuollossa ei viel\u00e4 ole vakiintunutta ty\u00f6kalua tunnistaa asiakkaan liikkumistottumuksia ennen asiakaskontaktia, mink\u00e4 avulla rajatuissa terveydenhuollon resursseissa voitaisiin kohdentaa liikkumisen puheeksi otto ja palveluohjaus riitt\u00e4m\u00e4tt\u00f6m\u00e4sti liikkuville. Vaikka fyysisen aktiivisuuden kyselylomakkeet korreloivat melko heikosti liikemittareiden kanssa liikkumisen ja paikallaanolon m\u00e4\u00e4r\u00e4n arvioinnissa, niill\u00e4 on monia etuja ja k\u00e4ytt\u00f6mahdollisuuksia fyysisen aktiivisuuden luokitteluv\u00e4lineen\u00e4. Luotettavien kyselylomakkeiden laatiminen vaatii runsaasti erilaista tutkimusta.\n\nT\u00e4m\u00e4n tutkimuksen tarkoituksena oli kehitt\u00e4\u00e4 seulontaty\u00f6kalu v\u00e4h\u00e4n liikkuvien tunnistamiseksi ja arvioida mahdollisuuksia integroida ty\u00f6kalu sosiaali- ja terveydenhuollon digitaaliseen asiakasportaaliin, mik\u00e4 auttaa sosiaali- ja terveydenhuollon ammattilaisia edist\u00e4m\u00e4\u00e4n t\u00e4llaisten asiakkaiden fyysist\u00e4 aktiivisuutta. Tutkimuksen tavoitteena oli validoida Helsingin asiakas- ja potilastietoj\u00e4rjestelm\u00e4 Apotin Maisa-asiakasportaaliin kehitett\u00e4v\u00e4 liikkumiskysely ja valittujen suomalaisten v\u00e4est\u00f6tutkimusten liikuntakysymykset liikemittarilla. Toisena tavoitteena oli l\u00f6yt\u00e4\u00e4 kysymysyhdistelm\u00e4, jolla voitaisiin ennustaa v\u00e4h\u00e4n liikkuva riitt\u00e4v\u00e4n luotettavasti.\n\n85 tutkittavaa k\u00e4ytti lantiolla pidett\u00e4v\u00e4\u00e4 liikemittaria seitsem\u00e4n p\u00e4iv\u00e4n ajan, mink\u00e4 j\u00e4lkeen he vastasivat fyysisen aktiivisuuden kyselylomakkeeseen. Yksitt\u00e4isten kysymysten luotettavuutta v\u00e4h\u00e4n liikkuvien henkil\u00f6iden tunnistamisessa arvioitiin korrelaatioanalyysill\u00e4. Logistisen regressioanalyysin avulla etsittiin kysymysyhdistelm\u00e4, joka tunnisti parhaiten v\u00e4h\u00e4n liikkuvat henkil\u00f6t. Paras kysymyspari loi \"liikennevalomallin\": punainen = v\u00e4h\u00e4n liikkuva molempien kysymysten perusteella, keltainen = v\u00e4h\u00e4n liikkuva toisen kysymyksen perusteella ja vihre\u00e4 = riitt\u00e4v\u00e4sti liikkuva molempien kysymysten perusteella. Malli osoitti, kuinka tarkasti kysymykset ennustavat henkil\u00f6n fyysisen aktiivisuuden.\n\nFyysisen aktiivisuuden seulontaan kehitetyll\u00e4 kyselylomakkeella on heikko kyky tunnistaa v\u00e4h\u00e4n liikkuvat henkil\u00f6t. Seulontakyselyn kysymysten ja liikemittarin korkein korrelaatio oli 0,46 paikallaanolon ja 0,42 kokonaisaktiivisuuden osalta. Kokonaisaktiivisuutta arvioivan parhaan kysymysparin mukaan 64 % kaikista v\u00e4h\u00e4n liikkuvista kuului \u201dliikennevalomallin\u201d punaiselle alueelle ja 36 % keltaiselle alueelle. Mallin herkkyys tunnistaa v\u00e4h\u00e4n liikkuvat punaiselle alueelle oli 64 % ja tarkkuus tunnistaa riitt\u00e4v\u00e4sti liikkuvat keltaiselle ja vihre\u00e4lle alueelle oli 82 %. T\u00e4ll\u00e4 tutkimusaineistoilla \u201dliikennevalomallin\u201d herkkyys ja spesifisyys eiv\u00e4t olleet riitt\u00e4v\u00e4n korkeita, jotta t\u00e4t\u00e4 l\u00e4hestymistapaa voitaisiin k\u00e4ytt\u00e4\u00e4 kliinisess\u00e4 ymp\u00e4rist\u00f6ss\u00e4. In Finland, primary health care does not yet have a well-established low-cost and reliable tool to identify clients' PA habits, which could be used to target PA promotion and support services to those who are physically inactive, within limited health care resources. Although questionnaires correlate rather poorly with PA measures in assessing PA levels, questionnaires do have a place in situations where individuals are classified by their PA levels, and they have strengths in assessing PA. Developing reliable and valid questionnaires requires a lot of research.\n\nThe aim of this study was to develop a screening tool to identify physically inactive people and to evaluate possibilities to integrate this tool into a digital client portal for social services and health care to help health and social care professionals promote physical activity among these clients. The primary goal was to validate the PA screening questionnaires against accelerometer data. The secondary goal was to find a combination of questions that could predict physical inactivity with sufficient reliability.\n\nEighty-five subjects wore a hip-worn accelerometer for seven days, after which they answered to several PA questionnaires. The reliability of individual questions to identify physically inactive people was assessed by correlation analysis. Logistic regression analysis was used to find the combination of questions which best identified physically inactive persons. The best pair of questions created a \"traffic light\" model: red = physically inactive based on both questions, yellow = physically inactive based on the other question, and green = physically active based on both questions. The model showed how accurately the questions predict a person's PA.\n\nThe questionnaires developed for screening PA have a poor ability to find subjects who are physically inactive. The highest correlation of the screening questionnaire with the accelerometer was 0.46 for sedentary behaviour and 0.42 for total physical activity, respectively. The best pair of questions on total physical activity identified 64 % of all inactive subjects in a red zone and 36 % in a yellow zone. The model had a sensitivity of 64 % for the red zone and a specificity of 82 % for the yellow and green zones. With the present research data, the sensitivity and specificity of the traffic light model were not high enough so that this approach could be used in the clinical setting.
