Sähköistä asiointia kehittämällä kustannuksia voidaan alentaa ja palvelujen vaikuttavuutta lisätä. Aihe liittyy olennaisena osana laajaan sosiaali ja terveydenhuollon palvelurakenneuudistukseen, jonka kautta saavutetut tulokset ovat sekä julkisen sektorin talouden että väestön hyvinvoinnin kannalta kriittisiä. Sähköinen asiointi tarkoittaa julkisen hallinnon palvelujen käyttämistä tieto ja viestintätekniikan avulla. Tämä rajaa pois yksityisen sähköisen palvelutarjonnan. Sähköinen asiointi terveydenhoidossa on englanniksi egovernment in health care. Se on eri käsite kuin ehealth, joka kattaa myös yksityisen palvelutarjonnan ja viittaa laajempaan sähköisten terveyspalvelujen valikoimaan. Lähitulevaisuuden avainsana eri sähköisen asioinnin ratkaisujen välillä on integraatio. Sen toteutumista hidastaa kulttuurinmuutoksen kankeus. Terveydenhoidolla tarkoitetaan terveydenhoidon ammattilaisten toimintaa, joka on tarpeellista terveyden edistämisen, sairauksien ehkäisyn ja hoidon kannalta, sekä heidän palvelujaan potilaiden terveydentilan arvioimiseksi, säilyttämiseksi tai palauttamiseksi. Terveydenhoito ja terveydenhuolto ovat tämän tutkielman näkökulmasta synonyymejä. Organisaatiokulttuuri on yhteinen näkemys organisaation toimintatavoista. Se on perustavanlaatuinen käsitys siitä miten liiketoimintaa johdetaan ja miten työntekijöiden tulee käyttäytyä ja miten heitä tulee kohdella. Se nähdään yhteisenä oletusten ja käsitysten syvempänä tasona aina perusarvoja myöten. Toimintatapojen muutos on välttämätön, mutta mekanismien ymmärtämisessä on vakavia puutteita. Suurin ongelma ei ole tekninen, vaan kulttuurinen ja se on korjattavissa pitkäjänteisellä koulutuksella. Kouluttamisessa pyritään ensin vastaamaan kysymykseen MIKSI (by education) ja vasta sitten kysymykseen MITEN (by training). Tätä ei tietojärjestelmätieteen tutkimuksessa ole riittävästi huomioitu. This thesis explains the concept of e-government and how it is applied to health care. Also the links between the improvement of e-government in health care and organisational culture are clarified. The concepts of e-government, organisational culture and continuous improvement are highlighted by literature review and applied to a case study. It is assumed that by developing e-government costs can be reduced and the effectiveness of health services can be increased. This topic is an integral part of the on-going social welfare and health care reform the results of which are crucial for both the public sector economy and the well-being of the general public. The term e-government means the use of public sector services by means of information and communications technology. This definition rules out the private sector services. E-government in health care is not the same as e-health, which also covers the private service providers and in general refers to a wider range of electronic health services. In the near future integration will be the key concept encompassing the various technical solutions in this field. This progress is hindered by the slowness of cultural change. Health care refers to the services provided by health care professionals that are necessary for promoting health, treatment and prevention of illness and diseases. This includes their services to diagnose patients and to maintain or restore their health. The terms health care and health services are considered synonymous within this study. Organisational culture is a common view of the organisation's practices. It covers the fundamental understanding of how business is managed, how employees should behave and how they should be treated. It is a collection of assumptions and perceptions encompassing the core values of the organisation. Current practices have to change, but there are grave misconceptions about the mechanisms to make this happen. The main issue is not technical but cultural, and can only be fixed by persistent education and training. This requires first answering the question WHY (by education) and only then answering the question HOW (by training). This has not been sufficiently acknowledged in information systems research.
