Lis\u00e4ksi haluttiin selvitt\u00e4\u00e4 selk\u00e4ydinrefleksien ja tahdonalaisten\nmotoristen ohjausten k\u00e4ytt\u00e4ytymist\u00e4 tasapainokontrollissa. Tutkimukseen rekrytoitiin yhteens\u00e4\n9 koehenkil\u00f6\u00e4 (6 naista ja 3 miest\u00e4) joiden ik\u00e4haarukka oli 20\u201340 vuotta. Tutkimus koostui\nkahdesta mittauskerrasta. Mittauskertojen aikana tehtiin staattinen ja dynaaminen tasapainotesti\nvoimalevyll\u00e4, joista ker\u00e4ttiin EMG-raakadataa. Lis\u00e4ksi tutkittiin erikseen dynaamisen\ntasapainotestin aikaista EMG-aktiivisuutta, MEP-vasteita TMS:n sek\u00e4 H-refleksi\u00e4\ns\u00e4hk\u00f6stimulaation avulla. TMS kelan kiinnityksess\u00e4 k\u00e4ytettiin apuna uimalakkia mill\u00e4 saatiin\nkela kiinnitetty\u00e4 ja stabiloitua p\u00e4\u00e4laelle. Dynaamisissa tasapainotesteiss\u00e4 tuotettiin 16\u201322\neteen- ja taaksep\u00e4in suuntautunutta h\u00e4iri\u00f6t\u00e4 satunnaisessa j\u00e4rjestyksess\u00e4 3:lla eri\nstimulointiviiveell\u00e4. Stimulointiviivein\u00e4 k\u00e4ytettiin yksil\u00f6ityj\u00e4 laskennallisia viiveit\u00e4 0 ms, MLR\n(~55ms) ja MLR+120ms (~175ms) viiveit\u00e4. Stimulaatiot annettiin n\u00e4ill\u00e4 viiveill\u00e4\ntasapainoh\u00e4iri\u00f6n alusta.\nKahden mittauskerran v\u00e4linen toistettavuus (ICC) osoitti eteen suuntaan H-refleksin osalta\nolevan (ICC) erinomainen 0-viiveell\u00e4 (ICC>0,81), hyv\u00e4 (ICC=0,61\u20130,81) MLR-viiveell\u00e4 ja\nkeskinkertainen (ICC=0,41\u20130,60) MLR+120-viiveell\u00e4. Taakse suuntaan H-refleksin\nkorrelaatiot osoittivat erinomaista (ICC>0,81) 0- ja MLR+120-viiveill\u00e4 sek\u00e4 hyv\u00e4\u00e4 (ICC=0,61\u2013\n0,81) MLR-viiveell\u00e4. MEP-vasteiden toistettavuus eteen suuntaan oli erinomainen (ICC>0,81)\n0-viivell\u00e4 ja hyv\u00e4 (ICC=0,61\u20130,81) MLR+120-viiveell\u00e4. Taakse suuntaan MEP-vasteiden\ntoistettavuus osoitti hyv\u00e4\u00e4 (ICC=0,61\u20130,8) 0- ja MLR+120-viiveell\u00e4 ja erinomaista (ICC>0,81)\nMLR-viiveell\u00e4. H-refleksien osalta viiveiden v\u00e4lisiss\u00e4 vertailuissa h\u00e4iri\u00f6iss\u00e4 eteen 0- ja MLRviiveiden sek\u00e4 0- ja MLR+120ms -viiveiden v\u00e4lill\u00e4 havaittiin tilastollisesti merkitsev\u00e4 ero\n(p<0.001). H-refleksimittauksissa h\u00e4iri\u00f6ss\u00e4 taakse 0 ja MLR-viiveiden v\u00e4lill\u00e4 havaittiin\ntilastollisesti merkitsev\u00e4 ero (p=0.004). MEP h\u00e4iri\u00f6ss\u00e4 taakse 0 ja MLR-viiveiden (p=0,001) ja\n0- ja MLR+120ms -viiveiden (p<0,001) v\u00e4lill\u00e4 havaittiin tilastollisesti merkitsev\u00e4 ero. Tulokset\nosoittavat mittausten hyv\u00e4\u00e4 tai erinomaista luotettavuutta H-reflekseill\u00e4 ja MEP:lla taakse\nsuuntaan. Mittausten luotettavuus eteen suuntaan osoittaa H-reflekseill\u00e4 0-viiveell\u00e4\nerinomaista, MLR-viiveell\u00e4 hyv\u00e4\u00e4 ja MLR+120-viiveell\u00e4 keskinkertaista. MEP:n osalta eteen\nsuuntaan havaittiin erinomaista luotettavuutta 0-viiveell\u00e4 ja hyv\u00e4\u00e4 MLR+120-viiveell\u00e4. N\u00e4iden\ntulosten perusteella H-refleksi- ja TMS-mittaustapoja voidaan hy\u00f6dynt\u00e4\u00e4 tutkimusk\u00e4yt\u00f6ss\u00e4\nspinaalisen ja supraspinaalisen aktiivisuuden mittaamisessa dynaamisessa tasapainoh\u00e4iri\u00f6ss\u00e4.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "The main aim of this thesis was to examine the repeatability of Hoffmann reflex (H-reflex) and\nTranscranial Magnetic Stimulation (TMS) methods during fast postural perturbations. Spinal\nreflex and voluntary supraspinal activations were measured and analyzed in three different\nlatencies at both methods. Total of 9 participants (6 women, 3 men) between the ages of 20 and\n40 were recruited to the study. The study consisted of two separate measurements. In addition,\nEMG raw data was measured during dynamic balance test. The dynamic balance test consisted\nof at least 16-22 randomized anterior-posterior direction perturbations induced by motorized\nbalance platform. Same protocol of platform movement was used at dynamic balance tests and\nmeasurements with both H-reflex and TMS stimulations. Swim cap was used to place and\nstabilize the TMS coil on the head. Stimulation latencies were measured and calculated by\nindividual H-reflex latency. Same latencies were used in both H-reflex and TMS methods: 0-,\nMLR- (~80ms) and MLR+120ms (~200ms) from the start of the platform movement.\nRepeatability of two separate individual measurements in H-reflex to anterior direction showed\nexcellent (ICC>0,81) at 0-, good (ICC=0,61 \u2013 0,81) at MLR and moderate (ICC=0,41\u20130,60) at\nMLR+120 latency. In H-reflex measurements to posterior direction showed good (ICC=0,61 \u2013\n0,81) to excellent correlation (ICC>0,81). Repeatability of MEP latencies to anterior direction\nshowed good (ICC=0,61\u20130,81) correlation at MLR+120 latency and excellent correlation\n(ICC>0,81) at 0- latency. MEP measurements to posterior direction at 0- and MLR+120 showed\ngood (ICC=0,61\u20130,81) correlations and latency of MLR excellent (ICC>0,81) correlation.\nThere was significant difference between H-reflex latencies of 0 and MLR (p<0,001) and MLRMLR+120ms (p<0,001) to anterior direction. H-reflex to posterior direction showed significant\ndifference between 0 and MLR latencies (p<0,01). A significant difference to posterior\ndirection was observed between 0 and MLR (p<0,01) and between MLR and MLR+120ms\n(p<0,001). The results of this study addressed the reliability and repeatability of both H-reflex\nand TMS methods during dynamic weight-bearing conditions.
