Lis\u00e4ksi selvitet\u00e4\u00e4n, kuinka paljon uusia astmadiagnooseja brokodilataatiokokeen perusteella ilmenee ja eroavatko astmadiagnoosien m\u00e4\u00e4r\u00e4t eri osapopulaatioilla. Tutkielmassa vertaillaan populaatioiden v\u00e4lisi\u00e4 eroja riskilajien eli kest\u00e4vyysurheilulajien (j\u00e4\u00e4kiekko, uinti, maastohiihto, suunnistus ja luistelu) ja ei-riskilajien (salibandy, koripallo, jalkapallo, yleisurheilu ja voimistelu) harrastajia yhdess\u00e4 ja erikseen liikuntaseurassa harrastamattomien nuorten ryhm\u00e4\u00e4n. \n\nTutkimuksen aineistona on k\u00e4ytetty Terveytt\u00e4 edist\u00e4v\u00e4 liikuntaseura -pitkitt\u00e4istutkimuksen (TELS) ensimm\u00e4isen poikkileikkausajankohdan aineistoa, joka on ker\u00e4tty vuosina 2013\u20132014. Aineisto koostui virtaustilavuus-spirometriassa suoritetuista keuhkojen toimintakokeista eli spirometria- ja bronkodilataatiokokeista 15,47 \u00b1 0,55 \u2013vuotiailla nuorilla. Tutkittavia oli spirometriakokeissa 527 ja bronkodilataatiokokeissa 510. Tilastolliset analyysit suoritettiin IBM SPSS Statistics \u2013tilasto-ohjelman versiolla 26.0. Tulosanalyysiin k\u00e4ytettiin lineaarisen sekamallin analyysimenetelm\u00e4\u00e4 etsien pienint\u00e4 Bayesin informaatiokriteerin mukaista arvoa. Taustamuuttujien yhten\u00e4ist\u00e4mist\u00e4 varten muodostettiin painomuuttuja, jossa huomioitiin pituus, paino ja sukupuoli. Analyyseist\u00e4 poissuljettiin jo astmadiagnoosin saaneet ja astmal\u00e4\u00e4kityst\u00e4 k\u00e4ytt\u00e4v\u00e4t. Analyyseiss\u00e4 k\u00e4ytettiin 95 prosentin luottamusv\u00e4li\u00e4. \n\nRiskilajistatuksesta riippumatta liikuntaseurassa harrastavilla havaittiin absoluuttisesti suuremmat keuhkosuureiden arvot valtaosassa eri muuttujista verrattuna ei-liikuntaseuralaisiin, mutta tarkasteltaessa riskilajien edustajia ei-riskilajien edustajiin, oli riskilajien edustajilla pienemm\u00e4t keuhkofunktioiden arvot. Tilastollisesti merkitsevi\u00e4 eroavaisuuksia havaittiin liikuntaseuralaisten ja ei-liikuntaseuralaisten v\u00e4lill\u00e4 nopean vitaalikapasiteetin (FVC) (p=0,002) ja sekuntikapasiteetin (FEV1) (p=0,003) osalta. Tilastollisen merkitsevyyden rajoilla oli ei-liikuntaseuralaisten ja ei-riskilajien v\u00e4lill\u00e4 FVC (p=0,051) ja tilastollisesti merkitsev\u00e4n\u00e4 FEV1 (p= 0,043). Ei-riskilajien ja riskilajien v\u00e4lill\u00e4 oli tilastollisesti merkitt\u00e4v\u00e4 ero bronkolyyttivasteissa FEV1- (p=0,024) ja MEF50-muuttujissa (p=0,029). Astma-kriteerin t\u00e4ytt\u00e4v\u00e4 v\u00e4hint\u00e4\u00e4n 12 %:n ja 0,20 l bronkolyyttivaste FEV1- tai FVC-muuttujissa, todettiin ei-liikuntaseuralaisilla 4,9 %:lla, ei-riskilajien harrastajilla 6,5 %:lla ja riskilajien harrastajilla 8,4 %:lla. \n\nTutkimus osoitti, ett\u00e4 liikuntaseuratoimintaan osallistujilla oli suuremmat keuhkofunktioiden arvot, mutta kest\u00e4vyys- eli riskilajien edustajilla havaittiin yll\u00e4tt\u00e4en ei-riskilajien edustajia pienempi\u00e4 keuhkofunktioita, mik\u00e4 on ristiriidassa aiempien havaintojen kanssa. Aiemmin diagnosoimatonta astmaa l\u00f6ydettiin kaikissa osajoukkotarkasteluissa ja eniten liikuntaseuratoimintaan osallistuvilla nuorilla. Aiemmin diagnosoimattomien astmatapauksien m\u00e4\u00e4rien perusteella olisi aiheellista lis\u00e4t\u00e4 astman tunnistamiseen liittyv\u00e4\u00e4 tiedotusta sek\u00e4 nuorille, ett\u00e4 heid\u00e4n kanssaan vuorovaikuttavien toimijoiden keskuudessa, jotta astmaa osattaisiin tunnistaa paremmin ja yksil\u00f6t hakeutuisivat tehokkaammin hoidontarpeen arviointiin.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "The purpose of this thesis was to examine the differences of the pulmonary functions of adolescents who are participating in the Finnish sports clubs\u2019 activities and those who are not. In addition, the number of new asthma diagnoses emerge based on the bronchodilation test and whether the rates of asthma diagnoses differ in different subpopulations was also investigated. The thesis compares population differences between high-risk endurance sports (ice hockey, swimming, cross-country skiing, orienteering, skating) group to non-risk (floorball, basketball, football, track and field, gymnastics) group jointly and individually compared to the non-sport club participating group.\n\nThe data used in this thesis was collected in 2013 and 2014 as part of Health promoting Sports Club -study (HPSC). The data consisted of pulmonary function tests performed in flow-volume spirometry, including spirometry and bronchodilation tests in adolescents with a mean age of 15,47 \u00b1 0,55. Those already diagnosed with asthma and those taking asthma medication were excluded from the analyses The final sample consisted of 527 subjects with spirometry and 510 subjects with bronchodilatation tests. The statistical analyses were performed using version 26.0 of the IBM SPSS Statistics program. A linear mixed model analysis method was used for result analysis whilst using the smallest Bayesian information criteria value. A weight variable was established to homogenate background variables, considering height, weight, and gender. The analyses used a 95 % confidence interval.\n\nRegardless of risk group status, participants in sports clubs had absolute higher pulmonary values in most of the different variables compared to the non-participating group. When compared to the non-risk group, the risk group had lower values in most different pulmonary functions. Statistically significant differences were observed between sport club participating and non-participating groups for FVC (p=0.002) and FEV1 (p=0.003) values and statistically borderline significance was observed between non-risk and non-sport club groups at FVC (p=0.051). Statistically significant difference was observed in FEV1 (p= 0.043) between non-risk and risk groups as well as between non-risk and risk groups in FEV1% (p=0,024), MEF50% (p=0.029) and PEF% (p=0.036). New asthma diagnoses were made to 4.9 % of non-sport club, 6.5 % in non-risk and 8.4 % in risk group adolescents. \n\nThe study showed that participation in sport club hosted physical activities was associated with higher pulmonary functions, but representatives of the risk group did not have higher pulmonary function values than those of the non-risk group, which is not in line with previous findings. Previously undiagnosed asthma was found in all subset reviews and the most among adolescents in the endurance sport group.
