Suomessa fysioterapeutin suoravastaanoton toimintamalli on ollut perusterveydenhuollossa k\u00e4yt\u00f6ss\u00e4 reilun vuosikymmenen ajan ja toiminta on yleistynyt my\u00f6s maailmanlaajuisesti. Vaikka suoravastaanottotoiminnan hy\u00f6tyj\u00e4 on tutkittu monipuolisesti, ovat fysioterapeuttien omat \u00e4\u00e4net ja kokemukset toiminnasta j\u00e4\u00e4neet taka-alalle. T\u00e4m\u00e4n pro gradu -tutkielman tarkoituksena oli kartoittaa fysioterapeuttien kokemuksia suoravastaanottotoiminnasta ja -\nty\u00f6st\u00e4\u00e4n perusterveydenhuollon kontekstissa. Tutkimuksen tavoitteena oli lis\u00e4t\u00e4 ymm\u00e4rryst\u00e4 suoravastaanottotoimintaan liittyvist\u00e4 kokemuksista. Tietoa voidaan hy\u00f6dynt\u00e4\u00e4 suoravastanottotoiminnan ja koulutuksen kehitt\u00e4misess\u00e4.\n\nT\u00e4m\u00e4 laadullinen tutkimus toteutettiin hermeneuttisen tulkinnallisen fenomenologian l\u00e4ht\u00f6kohdista. Tutkimusaineisto ker\u00e4ttiin Etel\u00e4-Pohjanmaan ja Vaasan sairaanhoitopiirien alueelta perusterveydenhuollossa v\u00e4hint\u00e4\u00e4n vuoden suoravastaanottoty\u00f6t\u00e4 tehneilt\u00e4 fysioterapeuteilta (n=9) puolistrukturoitujen haastatteluiden avulla. Haastattelut toteutettiin verkkov\u00e4litteisesti (Microsoft Teams) 12/2020\u20133/2021 v\u00e4lisen\u00e4 aikana. Haastatteluista litteroitu tutkimusaineisto analysoitiin tulkinnallisen fenomenologisen analyysin (IPA)\nperiaatteita mukaillen.\n\nTutkimuksen analysoidusta aineistosta nousi esiin kolme kantavaa yl\u00e4teemaa;\nmoniammatillinen yhteisty\u00f6 toiminnan edellytyksen\u00e4, oma toimijuus suoravastaanoton kent\u00e4ss\u00e4 sek\u00e4 suoravastaanottoty\u00f6ss\u00e4 kannattelevat ja horjuttavat tekij\u00e4t. N\u00e4iden yl\u00e4teemojen alle muodostui kahdeksan aineistoa kuvailevaa alateemaa. Fysioterapeutit kokivat suoravastaanottoty\u00f6ns\u00e4 merkityksellisen\u00e4 sek\u00e4 potilaan ett\u00e4 terveydenhuoltoj\u00e4rjestelm\u00e4n kannalta. He luottivat omaan ammattitaitoonsa ja tapaansa tehd\u00e4 suoravastaanottoty\u00f6t\u00e4. Ty\u00f6ss\u00e4\nheit\u00e4 kannatteli kokemuksen tuoma varmuus, muiden sidosryhmien arvostus ja tuki sek\u00e4 jatkuvan kouluttautumisen mahdollisuudet. Suoravastaanottotoiminnassa koettiin olevan my\u00f6s haasteita ja kehitt\u00e4misen mahdollisuuksia. Toimintaa haastoi asiakkaiden vaihteleva ohjautuvuus suoravastaanottoon sek\u00e4 l\u00e4\u00e4k\u00e4rijohtoisen kulttuurin perinteet. Fysioterapeutit kokivat my\u00f6s j\u00e4\u00e4v\u00e4ns\u00e4 herk\u00e4sti irralleen moniammatillisesta ty\u00f6ryhm\u00e4st\u00e4\u00e4n ja toivat esiin kokemuksiaan oman aktiivisen esill\u00e4olon ja fyysisen l\u00e4heisyyden merkityksest\u00e4 suoravastaanottotoiminnan tunnetuksi tekemisen ja sujuvuuden kannalta.\n\nFysioterapeuttien kokemuksista k\u00e4sin suoravastaanoton toimintamalli voidaan n\u00e4hd\u00e4 l\u00e4ht\u00f6kohtaisesti onnistuneena esimerkkin\u00e4 ty\u00f6n uusjaosta ja yrityksest\u00e4 kohentaa asiakkaan hoitopolkua. Mallin toimivuudessa koettiin kuitenkin edelleen olevan ongelmakohtia, jotka on hyv\u00e4 huomioida toimintaa kehitett\u00e4ess\u00e4. Tutkimuksen tulokset tukevat aikaisempien laadullisten fysioterapeuttien kokemuksia k\u00e4sittelevien tutkimusten tuloksia, ja ne ovat merkityksellisin\u00e4 siksi, ett\u00e4 pelk\u00e4st\u00e4\u00e4n fysioterapeuttien omia suoravastaanottotoimintaan liittyvi\u00e4 kokemuksia on tutkittu maailmanlaajuisesti viel\u00e4 v\u00e4h\u00e4n.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "Direct access is a healthcare service model, which provides the client with an opportunity to seek physiotherapists without a general practitioners\u2019 referral. The direct access model has been operating in Finnish primary health care for over a decade and first point of contact physiotherapy have become more common worldwide. Despite the multifaceted benefits of direct access based on the global studies, physiotherapists\u2019 own experiences have remained in the background. The purpose of this master's thesis was to investigate primary health care\nphysiotherapists\u2019 experiences of direct access. The aim of this study was to increase the understanding of physiotherapists\u2019 own experiences concerning direct access. Findings of this study can be utilized for the development of direct access services and education in the future.\n\nIn this study, a qualitative study design based on hermeneutic interpretive phenomenological methodology was used. Research data was collected through semi-structured individual interviews. Primary care physiotherapists working in direct access (n=9) were interviewed from two Finnish hospital districts in western Finland. The interviews were conducted online (Microsoft Teams) between 12/2020-3/2021 by the authors. The research material transcribed from the interviews (80 pages) was analyzed according to the principles of interpretive\nphenomenological analysis (IPA).\n\nThree main themes were identified from the research material: multi professional cooperation as a foundation of direct access, physiotherapists\u2019 agency in the field of direct access, supporting and challenging elements of direct access. Eight sub themes were composed to describe findings under these three main themes. Physiotherapists perceived their direct access work meaningful and that was also the case from the perspectives of the clients and the health care system. Physiotherapists were confident of their own expertise and ability to perform in\ndirect access. Self-confidence related to work experience, the support and appreciation from the other healthcare stakeholders along with the possibilities to continuous education were described as the supporting factors of the direct access work. Also perceived challenges and improvement opportunities were identified in direct access services. Physiotherapists experienced client\u2019s varying referral to direct access and traditions of a doctor-led culture as challenges. Physiotherapists also experienced getting easily isolated from the multiprofessional team. They highlighted experiences of their own active presence and the significance of physical proximity to increase the general knowledge of direct access and the\nfluency of the service. \n\nFollowing by physiotherapists\u2019 own experiences, direct access model can be seen essentially as a successful example for the redistribution of the work and attempt to clarify clients\u2019 clinical pathways. However, challenges were still perceived with the functionality of the service that should be taken consideration when developing direct access model. Physiotherapists\u2019 experiences in this study support the findings of previous qualitative studies.
