Suomessa nuorten ei-toivottuja raskauksia on pyritty ehk\u00e4isem\u00e4\u00e4n muun muassa parantamalla ehk\u00e4isymenetelmien saatavuutta ja j\u00e4rjest\u00e4m\u00e4ll\u00e4 nuorille suunnattuja ehk\u00e4isypalveluja. Vaikka Suomessa nuorten raskaudet ovat v\u00e4hentyneet viimeisten vuosien aikana, vuonna 2020 tuhatta 15\u201319-vuotiasta kohden tehtiin yh\u00e4 6,4 raskaudenkeskeytyst\u00e4. Tutkimusten mukaan ei-toivottu raskaus on merkitt\u00e4v\u00e4 riski nuoren kokonaisvaltaiselle terveydelle riippumatta raskauden lopputulemasta. \n\nT\u00e4m\u00e4n tutkimuksen tarkoituksena oli selvitt\u00e4\u00e4 suomalaisten 15-vuotiaiden raskauden ehk\u00e4isyn k\u00e4ytt\u00f6\u00e4 sek\u00e4 siihen yhteydess\u00e4 olevia yksil\u00f6- ja perhetekij\u00f6it\u00e4. Tutkimuksen aineistona hy\u00f6dynnettiin vuonna 2014 suomalaisissa kouluissa ker\u00e4tty\u00e4 WHO-Koululaistutkimuksen kyselyaineistoa 9.-luokkalaisten tutkittavien (n=2476) osalta. Aineisto analysoitiin IBM SPSS Statistics 27 -ohjelmistolla k\u00e4ytt\u00e4en ristiintaulukointia, Khiin neli\u00f6 -testi\u00e4, bin\u00e4\u00e4rist\u00e4 logistista regressioanalyysia sek\u00e4 riippumattomien otosten t-testi\u00e4.\n\nTutkimukseen vastanneista 9.-luokkalaisista 24,1 % raportoi olleensa sukupuoliyhdynn\u00e4ss\u00e4. Heist\u00e4 91,7 % raportoi k\u00e4ytt\u00e4neens\u00e4 luotettavaa raskauden ehk\u00e4isymenetelm\u00e4\u00e4 edellisess\u00e4 yhdynn\u00e4ss\u00e4\u00e4n. Yleisimmin k\u00e4ytetyt ehk\u00e4isymenetelm\u00e4t olivat kondomi ja e-pilleri. J\u00e4lkiehk\u00e4isy\u00e4 edellisess\u00e4 yhdynn\u00e4ss\u00e4\u00e4n kertoi k\u00e4ytt\u00e4neens\u00e4 10,6 % vastaajista, joskin ainoana ehk\u00e4isymenetelm\u00e4n\u00e4 sit\u00e4 k\u00e4ytettiin vain noin prosentissa tapauksista. Muita kuin edell\u00e4 mainittuja raskauden ehk\u00e4isymenetelmi\u00e4 raportoi k\u00e4ytt\u00e4neens\u00e4 5,5 % vastaajista. Raskauden ehk\u00e4isyn laiminly\u00f6minen oli tavallisempaa, mik\u00e4li nuori oli ollut ensimm\u00e4isess\u00e4 sukupuoliyhdynn\u00e4ss\u00e4\u00e4n hyvin nuorena, h\u00e4nell\u00e4 oli heikko terveyden lukutaito, tai nuoren perheen varallisuustaso oli matala.\n\nT\u00e4m\u00e4n tutkimuksen perusteella voidaan todeta, ett\u00e4 useimmat 9.-luokkalaiset suomalaisnuoret k\u00e4ytt\u00e4v\u00e4t raskauden ehk\u00e4isy\u00e4 ollessaan sukupuoliyhdynn\u00e4ss\u00e4. Toisaalta aikaisemmissa tutkimuksissa nuorille suositeltujen pitk\u00e4kestoisten raskauden ehk\u00e4isymenetelmien sek\u00e4 kaksoisehk\u00e4isyn k\u00e4ytt\u00f6 suomalaisnuorilla vaikuttaa olevan v\u00e4h\u00e4ist\u00e4. Nuorten hyv\u00e4 terveyden lukutaito voi raskauden ehk\u00e4isyn k\u00e4yt\u00f6n my\u00f6t\u00e4 suojata ei-toivottujen raskauksien aiheuttamilta terveyshaitoilta. Yhdenvertaisuuden toteutumiseksi raskauden ehk\u00e4isyn k\u00e4ytt\u00f6\u00e4 tulisi pyrki\u00e4 lis\u00e4\u00e4m\u00e4\u00e4n erityisesti ryhmiss\u00e4, joissa sen k\u00e4ytt\u00f6 on v\u00e4h\u00e4ist\u00e4 esimerkiksi yksil\u00f6llisell\u00e4 ja oikea-aikaisella tuella ja neuvonnalla sek\u00e4 maksuttoman ehk\u00e4isyn tarjoamisella. Jatkotutkimusta tarvitaan selvitt\u00e4m\u00e4\u00e4n pitk\u00e4kestoisten raskauden ehk\u00e4isymenetelmien v\u00e4h\u00e4iseen k\u00e4ytt\u00f6asteeseen liittyvi\u00e4 tekij\u00f6it\u00e4 sek\u00e4 sit\u00e4, voitaisiinko pitk\u00e4kestoisten menetelmien k\u00e4ytt\u00f6\u00e4 lis\u00e4\u00e4m\u00e4ll\u00e4 ehk\u00e4ist\u00e4 ei-toivottuja raskauksia nuorten keskuudessa nykyist\u00e4 tehokkaammin.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "Unwanted pregnancies among adolescents are a major global health concern. In Finland, efforts have been made to prevent these unwanted pregnancies by increasing the availability of contraceptive methods and by arranging contraceptive services targeted for adolescents. Even though the number of adolescent pregnancies in Finland has decreased in recent years, in 2020 there were still 6,4 abortions carried out per thousand 15\u201319-year-olds. Studies show that an unwanted pregnancy is a significant risk to an adolescent\u2019s overall health, no matter the outcome of the pregnancy.\n\nThe aim of this study was to review the use of contraception in Finnish 15-year-olds and the individual and family factors related to it. The data used in this study was a sub-data of the Health Behaviour of School-aged Children (HBSC) questionnaire study conducted in 2014. The sub-data comprised of the answers of Finnish 9th-graders (n=2476). The data was analyzed using IBM SPSS Statistics 27 -software. Cross-tabulation, Chi-square test, binary logistic regression, and independent samples T test were used as methods of analysis.\n\n24,1 % of the 9th-graders who responded, reported that they had had sexual intercourse. 91,7 % of them reported using a reliable method of contraception at their last intercourse. The most commonly used methods of contraception were a condom and an e-pill. The use of emergency contraception was reported by 10,6 % of respondents, although, it was used as the only method of contraception in only about 1 % of the cases. 5,5 % of respondents reported that they used other methods of contraception. It was more common not to use contraception if the adolescent had had their first sexual intercourse at a very young age, had poor health literacy, or the adolescent\u2019s family was categorized as having a low level of wealth.\n\nBased on this study, it can be stated that majority of Finnish 9th-graders use contraception when having sexual intercourse. On the other hand, the use of dual contraception as well as the use of long-term contraception (LARC), which is recommended for adolescents in previous studies, is rare. With a higher level of contraception use, good health literacy could protect against the negative health effects of unwanted pregnancies. In order to achieve equity, efforts should be made to increase the use of contraception among groups where its use is low. This could for example be done by providing adolescents with individual and timely support and counseling as well as by providing free of charge contraception for adolescents.
