Fyysisen ty\u00f6kuormituksen on todettu olevan yhteydess\u00e4 tuki- ja liikuntaelinsairauksiin, laitoshoidon tarpeen kasvuun sek\u00e4 ennenaikaiseen kuolemaan. Tutkielmassa selvitettiin, miten keski-i\u00e4n ty\u00f6n fyysiset kuormitustekij\u00e4t ovat yhteydess\u00e4 miesten my\u00f6hemm\u00e4n el\u00e4m\u00e4n fyysisen toimintakyvyn kehitykseen ja vaikuttivatko ammattiasema ja fyysinen aktiivisuus keski-i\u00e4n ty\u00f6n fyysisen kuormituksen ja fyysisen toimintakyvyn kehitykseen. \n\nTutkielman aineistona k\u00e4ytettiin Helsingin syntym\u00e4kohorttitutkimuksen aineistoa vuosilta 2001\u20132018. Tutkimukseen osallistui 928 miest\u00e4, joiden keski-ik\u00e4 vuonna 2001 oli 61,5 vuotta. Fyysisen toimintakyvyn kehityksest\u00e4 muodostettiin aineistol\u00e4ht\u00f6isesti kolme trajektoria Short-Form Survey 36-kyselyn (SF-36) fyysisen toimintakyvyn tulosten perusteella. Keski-i\u00e4n ty\u00f6n fyysiset kuormitustekij\u00e4t perustuivat tutkittavien ammattinimikkeisiin V\u00e4est\u00f6rekisterin vuoden 1990 ammattiluokitustietojen mukaisesti ja ne validoitiin sukupuolispesifill\u00e4 Ty\u00f6terveyslaitoksen ty\u00f6altistematriisilla (Job Exposure Matrix). Tutkitut ty\u00f6n fyysiset kuormitustekij\u00e4t olivat fyysisesti raskas ty\u00f6, polvillaan ty\u00f6skentely, ajaminen, raskas nostaminen, ty\u00f6skentely k\u00e4det hartialinjan yl\u00e4puolella, huono ryhti, nostaminen, k\u00e4sivoima, toistuvat k\u00e4siliikkeet, istuminen ja seisominen. N\u00e4ist\u00e4 muodostettiin faktorianalyysin avulla kaksi faktoria, joista laskettiin summamuuttujat hankala ty\u00f6asento ja -liikkeet sek\u00e4 fyysinen voimank\u00e4ytt\u00f6 ja ajaminen. Summamuuttujien yhteytt\u00e4 miesten my\u00f6hemm\u00e4n el\u00e4m\u00e4n fyysiseen toimintakykyyn tutkittiin multinomiaalisella regressioanalyysill\u00e4. \n\nMolemmat summamuuttujat hankala ty\u00f6asento ja -liikkeet sek\u00e4 fyysinen voimank\u00e4ytt\u00f6 ja ajaminen madalsivat todenn\u00e4k\u00f6isyytt\u00e4 kuulua parhaan fyysisen toimintakyvyn trajektoriin verrattuna heikoimman fyysisen toimintakyvyn trajektoriin (OR 0,997, 95 % LV 0,996\u20130,999 p = 0,002), (OR = 0,996, 95 % LV 0,992\u20131,000 p = 0,030). Kun malli vakioitiin ammattiasemalla, yhteys ei s\u00e4ilynyt tilastollisesti merkitsev\u00e4n\u00e4 ja se heikentyi edelleen, kun malliin lis\u00e4ttiin fyysinen aktiivisuus. Ammattiaseman vaikutus keski-i\u00e4n ty\u00f6n fyysisen kuormituksen ja miesten fyysisen toimintakyvyn my\u00f6hemm\u00e4n el\u00e4m\u00e4n kehitykseen oli merkitsev\u00e4. Fyysisen aktiivisuuden vaikutus t\u00e4h\u00e4n yhteyteen oli merkitsev\u00e4 runsaimman m\u00e4\u00e4r\u00e4n (3\u20137/vko) osalta. \n\nKeski-i\u00e4n fyysisen ty\u00f6kuormituksen yhteys heikompaan my\u00f6hemm\u00e4n el\u00e4m\u00e4n fyysisen toimintakyvyn kehitykseen ei s\u00e4ilynyt, kun malli vakioitiin ammattiasemalla. Miesten keski-i\u00e4n ty\u00f6n fyysiseen kuormitukseen tulisi kiinnitt\u00e4\u00e4 huomiota erityisesti ty\u00f6ntekij\u00f6ill\u00e4 toimintakykyisten elinvuosien turvaamiseksi, sek\u00e4 selvitt\u00e4\u00e4 fyysisen toimintakyvyn kannalta fyysisen ty\u00f6n kuormituksen haitallisia kynnysarvoja.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "Physical job strain, such as heavy dynamic muscle work, back rotations, similar repetitive movements, and static muscle work in poor working postures, have been found to be associated with changes in functional capacity later in life. Physical job strain has been linked to musculoskeletal disorders, increased need for institutional care and premature death. The aim of the study was to investigate how physical job strain in midlife is related to the development of physical functioning later in life in men and whether occupational status and physical activity influenced the development of physical job strain and physical functioning in middle-aged work.\n\nThe data of this study belong to the Helsinki Birth Cohort Study (HBCS) from 2001\u20132018. The study involved 928 men, with a mean age of 61.5 years in 2001. Three trajectories of the development of physical functioning were formed based on the results of the physical functioning of the SF-36 survey. The physical job strain factors in midlife were based on the occupational titles of the subjects according to the 1990 occupational classification data of the Population Register and were validated using the gender-specific occupational exposure matrix (JEM) of the Finnish Institute of Occupational Health. The physical job strain factors studied were physically strenuous work, kneeling, driving, heavy lifting, working hands above shoulders, awkward posture, lifting, hand force, repetitive hand movements, sitting and standing. From these, two factors were formed using factor analysis, from which the sum variables difficult work position and movements and the use of physical force and driving were calculated. The association between the sum variables and the physical functioning at later life in men was examined by multinomial regression analysis.\n\nBoth sum variables, awkward working position and movements, and physical force and driving reduced the probability of being on the trajectory of the best physical performance compared to the trajectory of the weakest physical performance (OR 0.997, 95% CI 0.996\u20130.999 p = 0.002), (OR = 0.996, 95% CI 0.992\u20131,000 p = 0.030). When the model was adjusted by a social class the connection did not remain statistically significant and further deteriorated when physical activity was added to the model. The impact of the social class on the development of the physical job train in midlife and the physical functioning later life in men was significant.\n\nThe association between the physical job strain in midlife and the weaker development of physical functioning later in life was not preserved when the model was standardized by the social class.
