Terveyden ja hyvinvoinnin laitoksen FinTerveys 2017 -tutkimuksessa edellisten 30 p\u00e4iv\u00e4n aikana alaselk\u00e4kipua oli kokenut naisista 48 % ja miehist\u00e4 44 %. Terapeuttisella harjoittelulla on todettu olevan vaikutusta toimintakyvyn parantumiseen sek\u00e4 oireiden lievittymiseen alaselk\u00e4kivun hoidossa. Etenkin keskivartalon lihaksia ja toimintakyky\u00e4 progressiivisesti kehitt\u00e4v\u00e4 voimaharjoittelu on todettu tehokkaaksi menetelm\u00e4ksi pitkittyneen alaselk\u00e4kivun hoidossa. \nT\u00e4m\u00e4n tutkimuksen tarkoituksena oli selvitt\u00e4\u00e4, mitk\u00e4 keskivartalon lihaksia vahvistavista harjoitteista oli tehokkaimpia eli aktivoi lihaksia eniten. Tutkimukseen osallistuneet vapaaehtoiset (n=9) olivat terveit\u00e4, keski-i\u00e4lt\u00e4\u00e4n 28-vuotiaita miehi\u00e4. Tutkimuksessa tarkasteltiin kuuden eri keskivartalolihaksen aktiivisuutta kymmeness\u00e4 eri harjoitteessa pinta-elektromyografilla (EMG) mitattuna. Tuloksissa harjoitteiden lihasaktiivisuus suhteutettiin prosentuaaliseksi osuudeksi tutkittavien maksimaalisesta isometrisest\u00e4 voimantuotosta (%MVIC). Tulokset esitet\u00e4\u00e4n keskiarvoina ja keskihajontana. \nTilastollisen analyysin p\u00e4\u00e4tuloksen perusteella harjoitteiden v\u00e4lill\u00e4 oli tilastollisesti merkitsevi\u00e4 eroja (p< 0.001) kun tarkasteltiin yksitt\u00e4isten lihasten tuottamaa lihasaktiivisuutta. Yksitt\u00e4isi\u00e4 lihaksia tarkastellessa m. rectus abdominikselle tehokkain harjoite oli lankku vartalon kierrolla (11. 9\u00b1 4.7 %MVIC), kuten my\u00f6s m. external obliquelle (20.8\u00b1 9.3 %MVIC). M. internal obliqueta eniten aktivoi ristitalja, jossa suurempi lihasaktiivisuus etummaisen eli vet\u00e4v\u00e4n k\u00e4den puolella (16.7\u00b1 6.9 %MVIC). M. iliopsoakselle tehokkain harjoite oli jalan taaksevienti alataljassa, jossa tukijalan puoli tuotti suuremman lihasaktiivisuuden (26. 7\u00b1 16.7 %MVIC). Maastaveto oli tehokkain harjoite niin m. lumbar erector spinaelle (47.7\u00b1 24.6 %MVIC) kuin m. multifidukselle (35.6\u00b1 16.5 %MVIC). Kaikkien lihasten aktiivisuustasojen keskiarvojen mukaan maastaveto oli tehokkain harjoite (19.6 \u00b1 12.3 %MVIC).\nTutkimuksen tulokset ovat samansuuntaisia aikaisempien tutkimusten kanssa, joissa on raportoitu maastavedon ja vaakapenkin olevan tehokkain harjoite selk\u00e4lihaksille. Vatsan puolen lihaksille on useassa tutkimuksessa tehokkaimmaksi harjoitteeksi raportoitu lankun erilaiset variaatiot, erityisesti kylkilankku. T\u00e4m\u00e4n tutkimuksen tulokset voivat olla hy\u00f6dyllisi\u00e4 fysioterapeuteille alaselk\u00e4kuntoutuksen ohjauksessa ja harjoitteiden valinnassa, kun edet\u00e4\u00e4n alhaisen kuormitustason harjoittelusta progressiivisesti lihaksia enemm\u00e4n kuormittaviin harjoitteisiin, kuntoutujan yksil\u00f6lliset tarpeet huomioiden.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "Low back pain is globally one of the most common reasons for sick leaves. In the last 30 days 48% of women and 44% of men had experienced low back pain according to the FinTerveys 2017 study done by the Finnish Institute for Health and Welfare. In treating low back pain, therapeutic exercise has been found to have an impact on physical performance and alleviating the symptoms. Especially progressive strength and functional training has been found to be an effective way to treat low back pain. \nThe purpose of this study was to determine and compare the activity levels of the core musculature during the exercises. The participants were voluntary, physically active (n=9) healthy adult males, aged 28 years on average. Muscle activity was measured by surface electromyography from six core muscles during ten different exercises, and it is presented as electrical muscle activity in relation to maximal voluntary isometric contraction (%MVIC). The values presented are means and standard deviations (SD). \nAccording to the statistical analyses, the differences between the activity levels of the exercises were shown to be statistically significant (p<0.001). The highest average EMG-value from all of the trunk muscles combined was observed during deadlift (19.6\u00b1 12.3 %MVIC). For m. rectus abdominis, the most efficient exercise was rotational plank (11.9\u00b1 4.7 %MVIC) as well as for m. external oblique (20.8\u00b1 9.3 %MVIC). M. internal oblique produced the highest average muscle activity in cable push and pull, in which the side that was pulling produced higher muscle activity (16.7\u00b1 6.9 %MVIC). For m. iliopsoas, the highest muscle activity was during the cable hip extension on the grounded supporting leg (26.7\u00b1 16.7 %MVIC). Deadlift was the most efficient exercise for m. lumbar erector spinae (47.7\u00b1 24.6 %MVIC) and m. multifidus (35.6\u00b1 16.5 %MVIC). \nThese results are in alignment with previous studies, where deadlift and lumbar extension bench were reported to produce the highest levels of muscle activity in the lumbar muscles. For abdominal muscles, different plank variations, especially side plank, has been reported to produce significantly higher muscle activation compared to other exercises.
