S\u00e4dehoidon tarkoitus on tappaa sy\u00f6p\u00e4solut tervekudosta s\u00e4\u00e4st\u00e4en. VMAT-tekniikalla saadaan aikaan tarkasti hoitokohteen muotoinen annosjakauma, mink\u00e4 takia potilaan asetteluun t\u00e4ytyy kiinnitt\u00e4\u00e4 erityist\u00e4 huomiota, jotta hoitokohde saa riitt\u00e4v\u00e4n suuren s\u00e4teilyannoksen ja hoitokohdetta ymp\u00e4r\u00f6iv\u00e4t elimet s\u00e4\u00e4styv\u00e4t optimaalisesti s\u00e4teilyn aiheuttamilta haitoilta.\n\nUseat modernit s\u00e4dehoitolaitteet on varustettu hoitop\u00f6ydill\u00e4, jotka mahdollistavat potilaan kiertojen korjaamisen. Tutkielman tarkoituksena oli selvitt\u00e4\u00e4, miten potilaan kierrot vaikuttavat annosjakaumaan, miss\u00e4 tilanteissa kiertoja kannattaisi kompensoida hoitop\u00f6yd\u00e4n kierroilla ja onko jokin potilasryhm\u00e4 erityisen herkk\u00e4 kiertojen vaikutuksille.\n\nKiertojen merkityst\u00e4 annosjakaumaan tutkittiin kiert\u00e4m\u00e4ll\u00e4 20 rintasy\u00f6p\u00e4potilaan annossuunnittelu-CT-kuvia \u00b13\u25e6 asti kolmen toisiaan vastaan kohtisuoran akselin ymp\u00e4ri. Alkuper\u00e4isen s\u00e4dehoitosuunnitelman tuottama annosjakauma lasketettiin kierretyille CT-kuvapakoille. Alkuper\u00e4isest\u00e4 ja kierretyst\u00e4 annosjakaumasta m\u00e4\u00e4ritettiin annostilavuusparametreja, jotka kuvaavat hoitokohteen ja sit\u00e4 ymp\u00e4r\u00f6ivien elimien annosjakaumaa. Parametreja analysoitiin tilastollisesta ja kliinisest\u00e4 n\u00e4k\u00f6kulmasta.\n\nTulosten perusteella jopa \u00b11\u25e6 kierroilla voi olla tilastollisesti ja kliinisesti merkitt\u00e4vi\u00e4 vaikutuksia suurimpaan annokseen yhdelle kuutiosenttimetrille syd\u00e4mess\u00e4. Lis\u00e4ksi \u00b12\u25e6 kierrot voivat vaikuttaa negatiivisesti useiden annos-tilavuusparametrien arvoihin. T\u00e4m\u00e4n takia \u00b11\u25e6 ja sit\u00e4 suurempien kiertojen korjaamista suositellaan. Mik\u00e4\u00e4n potilasryhm\u00e4 ei ollut merkitt\u00e4v\u00e4sti herkempi kiertojen vaikutuksille kuin muut.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "In this Master\u2019s Thesis project, the effect of patient rotations on the dose distribution in Volumetric Modulated Arc Therapy (VMAT) treatment of a left-sided breast cancer patient was studied. The objective of radiation therapy is to kill cancer cells while sparing the healthy tissue. With the VMAT technique, the acquired dose distribution is conformal to the treatment target. Thus, one has to pay special attention to the patient set-up to ensure that the target is covered by an adequate dose and the surrounding organs are spared optimally from the effects of radiation.\n\nModern radiotherapy treatment machines can be equipped with robotic treatment couches which enable compensating rotational set-up errors. The aim of this thesis was to find out how rotations affect the planned dose distribution, in which cases the set-up errors should be corrected using the robotic couch and if one group of patients is more sensitive to rotations than the others. \n\nThe impact of rotations on the dose distribution was studied by rotating the planning CT stacks of 20 breast cancer patients by angles up to \u00b13\u25e6 around three perpendicular axes. The dose distribution produced by the original treatment plan was calculated for the rotated CT image stacks. Dose-volume parameters, which describe the dose distribution of the treatment target and surrounding organs, were determined for the original and rotated images and analysed both from statistical and clinical perspective. \n\nThe results indicate that rotational set-up errors of \u00b11\u25e6 can have statistically and clinically significant effects on the dose to the cubic centimeter of the heart receiving the largest dose. Additionally, rotations by \u00b12\u25e6 can cause significant degradation on several dose-volume parameter values. Thus, it is recommended that rotations by \u00b11\u25e6 or more are corrected.
