Sen kriteereiss\u00e4 on kasvanut vy\u00f6t\u00e4r\u00f6nymp\u00e4rys ja kohonnut verenpaine sek\u00e4 poikkeavat veren sokeri- ja rasva-arvot. Riskitekij\u00e4n\u00e4 on erityisesti lihavuus keskivartalossa, jonne ylim\u00e4\u00e4r\u00e4inen energia varastoidaan elimist\u00f6n normaalien varastojen ollessa t\u00e4ynn\u00e4. T\u00e4h\u00e4n liittyy my\u00f6s insuliini, joka m\u00e4\u00e4ritt\u00e4\u00e4 elimist\u00f6n energiank\u00e4ytt\u00f6\u00e4. Elimist\u00f6n kyky\u00e4 vaihtaa energianl\u00e4hteit\u00e4 tilanteen mukaan kutsutaan aineenvaihdunnalliseksi joustavuudeksi, mutta insuliiniresistenteill\u00e4 sek\u00e4 ylipainoisilla se on h\u00e4iriintynyt. Syin\u00e4 pidet\u00e4\u00e4n mm. fyysist\u00e4 liikkumattomuutta ja rasvakudoksen toiminnan h\u00e4iri\u00f6it\u00e4. \nMetabolista oireyhtym\u00e4\u00e4 sairastavien aikuisten henkil\u00f6iden aineenvaihdunnallista joustavuutta ja sen yhteytt\u00e4 paikallaanoloon sek\u00e4 fyysiseen aktiivisuuteen tarkasteltiin yhten\u00e4 osa-alueena ExSit-tutkimuksessa. Tutkittavien paikallaanoloa ja sen keskeytyst\u00e4 sek\u00e4 liikkumista mitattiin aktiivisuusmittarilla. Aineenvaihdunnallista joustavuutta arvioitiin kalorimetrian avulla hengityskaasuista laskemalla hengitysosam\u00e4\u00e4r\u00e4n (RQ) muutos paastosta insuliinistimulaatioon. Lis\u00e4ksi tutkittavat suorittivat maksimaalisen hapenottokykytestin. Tutkimuksessa tarkasteltiin my\u00f6s metabolisen oireyhtym\u00e4n osatekij\u00f6iden yhteyksi\u00e4 aineenvaihdunnalliseen joustavuuteen.\nMetabolista oireyhtym\u00e4\u00e4 sairastavien aineenvaihdunnallinen joustavuus oli negatiivisesti yhteydess\u00e4 sedentaariosuuden, paaston RQ:n ja rasituksen alimman RQ:n kanssa ja positiivisesti insuliinistimulaation aikaisen RQ:n kanssa (p<0.05). RQ:n muutos rasituksessa oli positiivisesti yhteydess\u00e4 kuntoon sek\u00e4 koko kehon massaan ett\u00e4 rasvattomaan massaan (p<0.05). Metabolisen oireyhtym\u00e4n osatekij\u00f6iden suhteen rasituksen alin RQ oli negatiivisesti yhteydess\u00e4 diastoliseen verenpaineeseen ja paaston RQ positiivisesti veren triglyserideihin (p<0.05). \nTulosten mukaan suurempi paikallaanolon m\u00e4\u00e4r\u00e4 liittyy huonompaan aineenvaihdunnalliseen joustavuuteen metabolista syndroomaa sairastavilla. Paastossa glukoosin lis\u00e4\u00e4ntynyt k\u00e4ytt\u00f6 liittyy korkeampaan veren triglyseridien m\u00e4\u00e4r\u00e4\u00e4n. Eli vaikka veress\u00e4 on paljon rasvaa, k\u00e4ytet\u00e4\u00e4n silti glukoosia. Excessive sedentary behavior even doubles the risk of developing metabolic syndrome, which is a multidimensional disease. Its criteria include increased waist circumference and high blood pressure, as well as abnormal blood sugar and fat values. A particular risk factor is abdominal obesity, where extra energy is stored when the body\u2019s normal stores are full. This also involves insulin, which determines the body's energy use. The body's ability to switch energy sources according to the situation is called metabolic flexibility, but in insulin-resistant and overweight people, it is impaired. The reasons are e.g. physical immobility and adipose tissue dysfunction.\nMetabolic flexibility in adults with metabolic syndrome and its correlation to sedentary behavior and physical activity were considered as one aspect of the ExSit study. The sedentary time and its interruption of the subjects and their physical activity were measured with an accelometer. Metabolic flexibility was assessed by calorimetry from respiratory gases by calculating the change in respiratory quotient volume (RQ) from fasting to insulin stimulation. In addition, subjects performed a maximal oxygen uptake test. The study also examined the correlations between metabolic syndrome components with metabolic flexibility.\nMetabolic flexibility in patients with metabolic syndrome was negatively correlated with sedentary rate, fasting RQ and lowest RQ of exercise, and positively with RQ during insulin stimulation (p <0.05). The change in RQ under exercise was positively related to fitness for both whole body mass and lean mass (p <0.05). Regarding the components of metabolic syndrome, the lowest RQ of exercise was negatively associated with diastolic blood pressure and fasting RQ was positively associated with blood triglycerides (p <0.05).\nThe results suggest that higher sedentary time is associated with impaired metabolic flexibility in patients with metabolic syndrome. Increased fasting glucose use is associated with higher blood triglycerides. That is, even though there is a lot of fat in the blood, glucose is still used.
