Perimenopausaaliseen vaiheeseen tultaessa kuukautiskierto muuttuu ep\u00e4s\u00e4\u00e4nn\u00f6lliseksi, hormonipitoisuudet veress\u00e4 muuttuvat ja estrogeenin v\u00e4heneminen alkaa vaikuttaa luustoon negatiivisesti. Perimenopaussin on m\u00e4\u00e4ritetty jatkuvan viel\u00e4 vuoden ajan viimeisen kuukautisvuodon j\u00e4lkeen. Koska liikunnalla tiedet\u00e4\u00e4n olevan positiivisia vaikutuksia luustoon, on t\u00e4rke\u00e4\u00e4 tutkia, onko voimantuotto-ominaisuuksien ja luun geometrian v\u00e4lill\u00e4 l\u00f6ydett\u00e4viss\u00e4 yhteyksi\u00e4. On olennaista huomioida my\u00f6s kehonkoostumuksen mahdollinen sekoittava vaikutus, koska sill\u00e4 tiedet\u00e4\u00e4n olevan yhteyksi\u00e4 luustoon. T\u00e4m\u00e4n tutkielman tarkoituksena oli selvitt\u00e4\u00e4, onko alaraajojen voimalla ja kehonkoostumuksella yhteytt\u00e4 s\u00e4\u00e4ri- ja reisiluun geometriaan perimenopausaalisilla naisilla.\nTutkielman aineisto on osa Jyv\u00e4skyl\u00e4n yliopiston Gerontologian tutkimuskeskuksen ERMA-tutkimuksen aineistoa. Tutkittavat olivat perimenopausaalisessa vaiheessa olevia naisia, i\u00e4lt\u00e4\u00e4n 47\u201355-v. (n=114). Poikkileikkausaineistossa s\u00e4\u00e4ri- ja reisiluun geometriaa mitattiin QCT-menetelm\u00e4ll\u00e4 diafyysien puoliv\u00e4list\u00e4 ja kehonkoostumus oli mitattu DXA-menetelm\u00e4ll\u00e4. Alaraajojen voimaa kuvaavat muuttujat olivat polven isometrinen ojennusvoima (KEF) ja vertikaalisen hypyn korkeus (VJH). S\u00e4\u00e4ri- ja reisiluun geometriaa mittaavia muuttujia olivat kuoriluun tiheys (CoD), kuoriluun poikkileikkauspinta-ala (CoA), SSI-arvo ja luun kokonaispoikkileikkauspinta-ala (ToA). Aineisto analysoitiin IBM SPSS Statistics 24-ohjelmalla. Muuttujien yhteyksi\u00e4 selvitettiin k\u00e4ytt\u00e4en Pearsonin korrelaatiokerrointa ja hierarkkista lineaarista regressiomallia, jossa mallissa 1 analysoitiin alaraajojen voiman yhteytt\u00e4 luumuuttujiin, ja malliin 2 lis\u00e4ttiin my\u00f6s kehonkoostumusmuuttujat: Kehon rasvaton massa (LM) ja rasvaprosentti (Fat %).\n\nKehon rasvattomalla massalla oli vahva yhteys kaikkiin luumuuttujiin, ja se oli t\u00e4rkein selitt\u00e4j\u00e4 luun geometrian vaihtelulle s\u00e4\u00e4ri- ja reisiluun diafyysien puoliv\u00e4liss\u00e4. My\u00f6s alaraajojen voimalla, erityisesti polven isometrisella ojennusvoimalla, oli kohtalainen tai heikko yhteys suurimpaan osaan luumuuttujista. Yhteydet olivat vahvempia s\u00e4\u00e4riluussa verrattuna reisiluuhun. Tutkielman tulokset osoittavat, ett\u00e4 kehonkoostumuksella on vahva yhteys luun geometriaan, rasvattoman massan ollessa t\u00e4rkein selitt\u00e4j\u00e4 painoa kantavien pitkien luiden diafyysien kuoriluun vahvuudelle. Rasvaprosentti ei v\u00e4ltt\u00e4m\u00e4tt\u00e4 ole yht\u00e4 merkitt\u00e4v\u00e4 tekij\u00e4 luuston kannalta perimenopausaalisessa vaiheessa. Alaraajojen voimalla ei ollut yhteytt\u00e4 kuoriluun tiheyteen, muuhun luun geometriaan yhteys havaittiin. T\u00e4m\u00e4 saattaa viitata siihen, ett\u00e4 alaraajojen voima on yksi tekij\u00e4, joka vaikuttaa luun vahvuuteen sen poikkileikkauspinta-alan koon kautta. Entering perimenopausal phase, menstrual cycle\nbecomes irregular, blood hormone concentrations change, and lower estrogen levels start to affect bones\nnegatively. Perimenopause is defined to continue for one year after the last menstrual bleeding. As\nexercise is known to have positive effect on bone structure, it is important to study if any associations\ncan be found between force production capabilities and bone geometry, considering also the possible\nconfounding association of body composition, which is known to have an effect on bone mass. The aim\nof this thesis was to explore, if associations exist between lower limb strength, body composition and\ntibial and femoral bone geometry in perimenopausal women.\nData of this cross-sectional thesis is part of the Estrogenic Regulation of Muscle Apoptosis (ERMA)\nstudy, by Gerontology Research Center, University of Jyv\u00e4skyl\u00e4. Participants were females in\nperimenopausal phase, aged 47-55 y (n=114). Data consisted of QCT-based bone geometry\nmeasurements made in mid-diaphyseal sites of tibia and femur, DXA-based body composition\nmeasurements and measurements of lower limb strength: Isometric knee extension force (KEF) and\nvertical jump height (VJH). Tibial and femoral bone geometry variables were cortical density (CoD),\ncortical area (CoA) of bone cross-section, Strength-Strain Index (SSI) and total area (ToA) of bone\ncross-section. Data was analyzed with IBM SPSS Statistics 24. Associations between variables were\nanalyzed using Pearson correlation and hierarchical linear regression model, where in model 1\nassociation of lower limb strength with bone geometry variables was analyzed, and in model 2 also body\ncomposition variables, lean mass (LM) and fat percentage (Fat %) were added into model.\nLean mass had strong associations with each bone geometry variable, being overall the most important\npredictor of bone geometry variation in tibial and femoral mid-diaphyseal sites. Also, lower limb\nstrength, especially knee extension force, had modest or weak associations with most bone geometry\nvariables, those being stronger in tibia than in femur. This thesis shows that body composition has strong\nassociations with bone geometry, lean mass appears as main predictor of cortical bone strength in middiaphyseal sites of weight-bearing long bones. Fat percentage may not be as significant factor in\nperimenopausal phase. Results indicate that lower limb strength may have a role in strengthening bones\nthrough associations to bone geometry other than cortical density. Further research is needed to clarify\ncause and effect-relations of lower limb strength and bone geometry.
