Sen vuoksi lis\u00e4\u00e4 tietoa tarvitaan esimerkiksi vaihdevuosien vaikutuksesta motoriseen toimintakykyyn sek\u00e4 taustalla vaikuttaviin mekanismeihin. T\u00e4m\u00e4n seurantatutkimuksen tarkoituksena oli transkraniaalista magneettistimulaatiota (TMS) hy\u00f6dynt\u00e4en tutkia keski-ik\u00e4isill\u00e4 naisilla vaihdevuosien etenemisen vaikutusta motorisen aivokuoren inhibitorisiin mekanismeihin ja aktivoitumisherkkyyteen. \nTutkimuksen aineisto on osa Estrogeeni, vaihdevuodet ja toimintakyky \u2013 ERMA-tutkimusta. Vaihdevuosistatus m\u00e4\u00e4ritettiin STRAW +10 -luokitusta mukaillen ja TMS-tutkimus toteutettiin perimenopaussin ja postmenopaussin aikana. TMS:n yksitt\u00e4ispulssistimulaatiolla tuotettiin motorinen vaste (MEP) sek\u00e4 silent period (SP) kolmella eri voimatasolla oikean nilkan isometrisen dorsiflexion aikana. EMG mitattiin tutkittavien oikean jalan etummaisesta s\u00e4\u00e4rilihaksesta. Lopullista analyysia varten 40 %:n ja 60 %:n voimatasot maksimaalisesta tahdonalaisesta lihassupistuksesta (MVC) yhdistettiin. Tilastomenetelmin\u00e4 k\u00e4ytettiin Wilcoxonin merkkitesti\u00e4 ja Spearmanin j\u00e4rjestyskorrelaatiokerrointa. \nTutkimus koostui 25 tutkittavasta, joiden seuranta-ajan keston mediaani oli 9,7 (IQR 9,5) kuukautta ja i\u00e4n mediaani loppumittauksissa 52,9 (IQR 3,5) vuotta. Seurannan aikana follikkelia stimuloivan hormonin (FSH) pitoisuus nousi (p=0,001) ja 17\u03b2-estradiolin (E2) pitoisuus laski (p=0,045) kuvastaen vaihdevuosien etenemist\u00e4. SP:n kestossa (p=0,615), MEP amplitudissa (p= 0,263) tai MVC:ssa (p=0,222) ei havaittu muutoksia alku- ja loppumittausten v\u00e4lill\u00e4. FSH:ssa ja E2:ssa tapahtuneet muutokset eiv\u00e4t korreloineet tilastollisesti merkitsev\u00e4sti SP:n kestossa, MEP amplitudissa tai MVC:ssa tapahtuneiden muutosten kanssa. \nT\u00e4m\u00e4n tutkimuksen perusteella vaihdevuosien eteneminen noin 10 kuukauden mittaisessa seurannassa ei vaikuta motorisen aivokuoren inhibitorisiin mekanismeihin tai aktivoitumisherkkyyteen SP:n kestolla ja MEP amplitudilla mitattuna. Pieni otoskoko sek\u00e4 muuttujissa havaittu runsas yksil\u00f6iden v\u00e4linen vaihtelu kuitenkin vaikeuttavat tulosten yleistett\u00e4vyytt\u00e4. Exposure to changes in sex hormones levels affects women\u2019s health and well-being. More information is needed, for example, how the menopausal transition affects motor function and the underlying physiological mechanisms. The purpose of this follow-up study was to investigate how the menopausal transition from perimenopause to postmenopause impacts middle-aged women\u2019s motor cortex properties such as cortical inhibition and excitability. \nThe data of this study is part of the multidisciplinary Estrogenic Regulation of Muscle Apoptosis (ERMA) study from Gerontology Research Center, University of Jyv\u00e4skyl\u00e4. The menopausal status of the participants was determined by slightly modified Stages of Reproductive Aging Workshop (STRAW) +10 guidelines. Transcranial magnetic stimulation (TMS) was conducted during perimenopause and postmenopause. Single-pulse TMS was used to elicit motor evoked potentials (MEP) and silent periods (SP) in the active tibialis anterior muscle during three different stages of maximal voluntary contraction (MVC). For the statistical analysis, 40 % and 60 % of MVC data were combined. Wilcoxon Signed Rank Test and Spearman\u2019s Rank Correlation were used for the analysis. \nThis study consists of 25 subjects with a median follow-up duration of 9.7 (IQR 9.5) months and a median age of 52.9 (IQR 3.5) years. During the follow-up, follicle-stimulating hormone (FSH) increased (p=0.001) and 17\u03b2-estradiol (E2) decreased (p=0.045) reflecting the progression of menopause. SP duration (p=0.615), MEP amplitude (p=0.263) or MVC (p=0.222) did not change during the follow-up. Changes in FSH or E2 did not correlate with changes in SP duration, MEP amplitude or MVC. \nThese data suggest that the progression of menopause at approximately 10 months of follow-up does not affect cortical inhibition and excitability as measured by SP duration and MEP amplitude. The large individual variability observed in the variables and the small sample size makes it difficult to generalize the results.
