T\u00e4m\u00e4n kuormittavuustutkimuksen tarkoituksena oli selvitt\u00e4\u00e4 eroaako kuntoutuspeliharjoittelu tavanomaisesta kotiharjoittelusta polvinivelen liikelaajuuden, kulmanopeuden ja kulmakertym\u00e4n osalta.\n\nTutkimukseen osallistuneet vapaaehtoiset henkil\u00f6t rekrytoitiin Keski-Suomen keskussairaalan polven tekonivelpoliklinikkak\u00e4ynneilt\u00e4. Pelilaitteistona k\u00e4ytettiin liikesensoria (Microsoft Kinect 2) ja tietokonetta, joka yhdistettiin televisioon. Kuntoutuspelit (Turku Game Lab) olivat Unity-pohjaisia, joista kaksi (Cave & Intruders) keskittyiv\u00e4t polven ekstensio- ja yksi (Rowing)\nfleksiosuunnan liikkeisiin. Pelit kehitettiin erityisesti polventekonivelleikkauksen j\u00e4lkeiseen kuntoutukseen. Tavanomaisina tekonivelleikkauksen j\u00e4lkeisin\u00e4 harjoitteina olivat koukistus istuen, koukistus seisten ja ojennus istuen. Polven nivelkulmaa ja kulmanopeutta mitattiin 2D-liikeanalyysin avulla. Liikelaajuus m\u00e4\u00e4riteltiin toistojen perusteella luokittelemalla ne liikeanalyysidatasta\nja tilastoanalyyseiss\u00e4 k\u00e4ytettiin painotettua keskiarvoa. Lis\u00e4ksi tarkasteltiin polvinivelen kulmanopeutta ja kulmakertym\u00e4\u00e4, joka m\u00e4\u00e4riteltiin kulmamuutoksen summana. Tulokset esitet\u00e4\u00e4n keskiarvona ja keskihajontana (\u00b1).\n\nTutkittavien (n = 7) mediaani-ik\u00e4 oli 66 (IQR = 10) vuotta. Istuen teht\u00e4v\u00e4n ojennusharjoitteen ekstensiovajaus 12(\u00b14)\u00b0 oli pienempi kuin Cave-pelin ekstensiovajaus 16(\u00b13)\u00b0 (p=0,028). Intruders-pelin ja ojennusharjoitteen v\u00e4lill\u00e4 ei ollut eroa. Fleksiosuunnan Rowing-pelin liikelaajuus 89(\u00b111)\u00b0 oli suurempi kuin seisten tehdyn koukistusharjoitteen liikelaajuus 86(\u00b112)\u00b0 (p=0,028). Istuen teht\u00e4v\u00e4n fleksioharjoitteen ja Rowing-pelin v\u00e4lill\u00e4 ei ollut eroa. Intruders-pelin kulmanopeus 51(\u00b122)\u00b0/s oli korkeampi kuin istuen teht\u00e4v\u00e4n ojennusharjoitteen kulmanopeus 23(\u00b113)\u00b0/s (p=0,046). Cave-pelin ja ojennusharjoitteen v\u00e4lill\u00e4 ei ollut eroa. Fleksiosuunnassa\nRowing-pelin kulmanopeus 84\u00b136 \u00b0/s oli korkeampi kuin koukistus seisten-harjoitteen 21(\u00b112)\u00b0/s (p=0,018). Rowing-pelin kulmanopeus oli my\u00f6s korkeampi kuin koukistus istuen\u2013harjoitteen kulmanopeus 16(\u00b110)\u00b0/s (p=0,018). Kulmakertym\u00e4 ei eronnut ekstensiosuunnan\nliikkeiss\u00e4 ja peleiss\u00e4. Fleksiosuunnassa Rowing-pelin kulmakertym\u00e4 4059(\u00b11932)\u00b0/min oli korkeampi kuin koukistus seisten-harjoitteen (2161(\u00b1463)\u00b0/min) (p=0,018). Rowing-pelin kulmakertym\u00e4 oli my\u00f6s korkeampi kuin koukistus istuen-harjoitteen (1039(\u00b1393)\u00b0/min) (p=0,018).\n\nKuntoutuspelien polvinivelelle tuottama liike t\u00e4ytt\u00e4\u00e4 harjoittelulle asetetut tavoitteet hyvin, vaikkakin se on luonteeltaan erilaista kuin tavanomainen kotiharjoittelu. This study preceded a later effectiveness study. The purpose of this study was to determine whether exergame training differs from therapeutical home training of knee joint mobility, angular velocity and angular accumulation.\n\nThe study participants were recruited from Central Hospital Central Finland. The game equipment consisted of a motion sensor (Microsoft Kinect 2) and a computer connected to a television. Exergames (Turku Game Lab) were Unity-based. Two of the games (Cave & Intruders)\nfocused on knee extension and one (Rowing) flexion movements. Exercises after knee replacement surgery included flexion while sitting, flexion while standing and extension while sitting. Knee joint angle and angular velocity were measured using 2D motion analysis. Mobility was determined by repetitions and a weighted average was used in statistical analyses. The angular\nvelocity was examined by the angular velocity and the total workload as the sum of the angular changes. Results are presented as mean and standard deviation (\u00b1).\n\nThe median age of the subjects (n=7) was 66 (IQR=10) years. Extension while sitting-exercise extension gap 10(\u00b16)\u00b0 was less than Cave-game extension gap 16(\u00b13)\u00b0 (p=0.028). There was no difference between Intruders-game and extension while sitting-exercise. The movement of the Rowing-game was 89(\u00b111)\u00b0 greater than the motion of the flexion while standing-exercise\n86(\u00b112)\u00b0 (p=0.028). There was no difference between flexion while sitting-exercise and Rowing-game. In the flexion direction, the angular velocity of the Rowing-game 84(\u00b136)\u00b0/s was higher than the flexion while standing-exercise 21(\u00b112)\u00b0/s (p=0.018). The angular velocity of the Rowing-game was also higher than the angular velocity of the flexion while sitting-exercise at 16(\u00b110)\u00b0/s (p=0.018). The total workload did not differ in extension directional movements and games. In flexion direction, the angular accumulation of the Rowing-game at 4059(\u00b11932)\u00b0/min was higher than the flexion while standing-exercise (2161(\u00b1463)\u00b0/min) (p=0.018). The angular accumulation of the Rowing-game was also higher than the flexion while sitting-exercise (1039(\u00b1393)\u00b0/min) (p=0.018).\n\nThe movement produced by exergames for the knee joint fulfils the training goals well.
