Se aiheuttaa yksil\u00f6lle merkitt\u00e4v\u00e4\u00e4 liikunta- ja toimintakyvyn heikentymist\u00e4. Yhteiskunnalle lonkkamurtumista aiheutuu huomattavia taloudellisia kustannuksia. Voimaharjoittelu on yleinen harjoittelumuoto lonkkamurtuman j\u00e4lkeen, ja sen on todettu lis\u00e4\u00e4v\u00e4n niin lihasmassaa kuin -voimaa. Tuoliltanousutesti on objektiivinen ja paljon k\u00e4ytetty toiminnallinen testi, jonka tuloksen on todettu parantuvan voimaharjoittelun seurauksena. T\u00e4m\u00e4n tutkielman tarkoituksena oli selvitt\u00e4\u00e4, vaikuttaako kolmen kuukauden ohjattu voimaharjoittelu lonkkamurtuman kokeneen ik\u00e4\u00e4ntyneen kehon lihasmassan m\u00e4\u00e4r\u00e4\u00e4n tai tuoliltanousu-testin tulokseen, ja onko kehon lihasmassan m\u00e4\u00e4r\u00e4ll\u00e4 yhteytt\u00e4 tuoliltanousutestin tulokseen.\n\nTutkimusaineisto oli osa satunnaistettua ja kontrolloitua HIP ASYMMETRY -tutkimusta. Tutkimukseen osallistui 60-85-vuotiaita naisia ja miehi\u00e4 (n=46), joiden lonkkamurtuma oli hoidettu Keski-Suomen keskussairaalassa vuosina 1998-2004. Tutkittavat satunnaistettiin harjoittelu- (n=24) ja kontrolliryhm\u00e4\u00e4n (n=22). Harjoitteluryhm\u00e4ss\u00e4 tehtiin alaraajojen nopeus- ja lihasvoimaharjoittelua kahdesti viikossa kolmen kuukauden ajan. Kontrolliryhm\u00e4l\u00e4isi\u00e4 kehotettiin jatkamaan normaalia el\u00e4m\u00e4\u00e4. Kehon lihasmassan m\u00e4\u00e4r\u00e4 arvioitiin bios\u00e4hk\u00f6isen impedanssin perusteella, ja tuolilta yl\u00f6snousua mitattiin viiden toiston tuoliltanousutestill\u00e4. Analyysit suoritettiin IBM SPSS Statistics 24 -ohjelmalla, ja tilastollisina menetelmin\u00e4 k\u00e4ytettiin Mann-Whitney U-testi\u00e4 ja Spearmanin korrelaatiokerrointa.\n\nHarjoitteluryhm\u00e4ss\u00e4 havaittiin suuntaa antavia muutoksia siit\u00e4, ett\u00e4 lihasmassan m\u00e4\u00e4r\u00e4 v\u00e4heni intervention aikana, kun kontrolliryhm\u00e4ss\u00e4 lihasmassan m\u00e4\u00e4r\u00e4 hieman kasvoi. Tuoliltanousutestin tuloksen suhteen molemmissa ryhmiss\u00e4 havaittiin liev\u00e4\u00e4 parantumista. Ryhmien v\u00e4lill\u00e4 ei kuitenkaan ollut tilastollisesti merkitsev\u00e4\u00e4 eroa lihasmassan m\u00e4\u00e4r\u00e4n tai tuoliltanousutestin tuloksen suhteen. Lis\u00e4ksi ainoastaan harjoitteluryhm\u00e4l\u00e4isill\u00e4 havaittiin lopputilanteessa heikko ja negatiivinen korrelaatio (r=-0,334) kehon lihasmassan m\u00e4\u00e4r\u00e4n ja tuoliltanousutestin tuloksen v\u00e4lill\u00e4, mutta yhteys ei ollut tilastollisesti merkitsev\u00e4.\n\nVoimaharjoittelulla ei ollut vaikutusta lihasmassan m\u00e4\u00e4r\u00e4\u00e4n tai tuoliltanousutestin tulokseen. Syyn\u00e4 saattaa olla se, ett\u00e4 tutkittavat olivat lonkkamurtumasta huolimatta suhteellisen terveit\u00e4 ja toimintakykyisi\u00e4. Lis\u00e4ksi on todenn\u00e4k\u00f6ist\u00e4, ett\u00e4 pelkk\u00e4 lihasmassan m\u00e4\u00e4r\u00e4 ei ole yhteydess\u00e4 fyysiseen toimintakykyyn. Keskeisempi tekij\u00e4 lienee lihaksen laatu eli lihasvoiman suhde \n lihasmassan m\u00e4\u00e4r\u00e4\u00e4n. Tulosten perusteella voimaharjoittelu soveltuu yksil\u00f6llisesti r\u00e4\u00e4t\u00e4l\u00f6ityn\u00e4 my\u00f6s hauraille ja i\u00e4kk\u00e4ille lonkkamurtumakuntoutujille. It causes major impairment of physical performance and mobility to an individual and high financial costs to the society. Resistance training is a common type of training after a hip fracture. Resistance training has been shown to increase muscle mass, muscle strength and muscle power. The sit-to-stand test is an objective and widely used functional test, and the result of the test has been shown to improve as a result of resistance training. The aim of this thesis was to study whether 12-week guided muscle strength and power training had an effect on muscle mass or the result of the sit-to-stand test in elderly with hip fracture as well as to investigate the associations between muscle mass and the result of the sit-to-stand test.\n\nThe research material was part of the randomized and controlled HIP ASYMMETRY study. The subject sample consisted of 60-85-year-old women and men (n=46) whose hip fracture had been operated in the Central Finland Central Hospital between 1998-2004. The\nparticipants were randomized into a training (n=24) and a control group (n=22). The training group did muscle strength and power training for lower limbs twice a week for three months. Controls were encouraged to continue their normal life. The muscle mass was estimated\nusing bioelectrical impedance, and the ability to rise from a chair was measured with five times sit-to-stand test. The data were analyzed using IBM SPSS Statistics 24 program, and the statistical methods used were Mann-Whitney U test and Spearman\u2019s correlation coefficient.\n\nThose in the training group seemed to lose a little bit of muscle mass whereas controls\u2019 muscle mass increased slightly during the intervention. The result of the sit-to-stand test was found to improve slightly in both groups. However, there was no statistically significant\ndifference between the training and the control groups with respect to muscle mass or the result of the sit-to-stand test. In addition, there was a weak negative correlation (r=-0,334) between the muscle mass and the result of the sit-to-stand test only in the training group after the intervention but the correlation wasn\u2019t statistically significant.\n\nThe resistance training had no effect on muscle mass or the result of the sit-to-stand test. The reason might be that the participants were relatively healthy and well-functioning despite the hip fracture. Furthermore, it is likely that muscle mass alone doesn\u2019t explain physical\nfunctioning. A more important factor is probably the quality of muscle. Based on the results, individually tailored resistance training is feasible also for the fragile elderly with hip fracture.
