Et\u00e4teknologiasta on haettu uusia keinoja v\u00e4est\u00f6n aktivoimiseen. T\u00e4m\u00e4n tutkielman tavoite oli selvitt\u00e4\u00e4 et\u00e4teknologiaa sis\u00e4lt\u00e4v\u00e4n kuntoutuksen vaikutuksia fyysisen aktiivisuuden eri rasittavuustasoihin (kevyt, keskiraskas, raskas ja kokonaisaktiivisuus) kuntoutujilla. Tutkielmassa pyrittiin my\u00f6s selvitt\u00e4m\u00e4\u00e4n, eroaako syd\u00e4n-, TULES- ja KIILA-kuntoutusryhm\u00e4t toisistaan l\u00e4ht\u00f6tilanteessa fyysisilt\u00e4, psykososiaalisilta tai demografisilta tekij\u00f6ilt\u00e4\u00e4n. Tavoitteena oli lis\u00e4ksi tarkastella kuntoutusryhmien v\u00e4lisi\u00e4 eroja kuntoutusjakson aikana havaittujen fyysisen aktiivisuuden muutosten osalta. Tutkielmassa tutkittiin my\u00f6s, mitk\u00e4 fyysiset ja psykososiaaliset tekij\u00e4t selitt\u00e4v\u00e4t mahdollista fyysisen aktiivisuuden lis\u00e4\u00e4ntymist\u00e4. Tutkimus oli osa laajempaa tutkimushanketta: Et\u00e4teknologian vaikuttavuus ja k\u00e4ytett\u00e4vyys Kelan tavanomaisessa kuntoutuksessa ja uudenlaisessa innovatiivisessa et\u00e4kuntoutuksessa. \n\nTutkimus oli prospektiivinen kohorttitutkimus, jossa kuntoutujat (N=36) saivat ohjausta kuntoutuskurssin l\u00e4sn\u00e4olokerroilla sek\u00e4 et\u00e4teknologian kautta. Kuntoutujien fyysist\u00e4 aktiivisuutta mitattiin aktiivisuusrannekkeella ennen kuntoutusta ja kuuden kuntoutuskuukauden j\u00e4lkeen. Tilastollisina menetelmin\u00e4 k\u00e4ytettiin pareittaisten otosten T-testi\u00e4, Wilcoxonin Signed Rank \u2013testi\u00e4, yksisuuntaista varianssianalyysi\u00e4, Kruskall-Wallisin testi\u00e4, Khiin neli\u00f6t\u00e4 ja lineaarista regressioanalyysi\u00e4. \n\nTutkielmassa havaittiin eroja kuntoutusryhmien v\u00e4lill\u00e4 l\u00e4ht\u00f6tilanteessa. TULES- ja syd\u00e4n-ryhm\u00e4t erosivat toisistaan kokonaisaktiivisuuden (p=0.009), kevyen aktiivisuuden (p=0.050) ja sukupuolijakauman (p=0.018) suhteen l\u00e4ht\u00f6tilanteessa. KIILA-ryhm\u00e4l\u00e4iset kokivat l\u00e4ht\u00f6tilanteessa sosiaalisen el\u00e4m\u00e4nlaatunsa heikommaksi kuin TULESryhm\u00e4l\u00e4iset (p=0.049) ja syd\u00e4n-ryhm\u00e4l\u00e4iset (p=0.036). Tutkielmassa todettiin et\u00e4teknologian vaikuttavan merkitsev\u00e4sti kuntoutujien kevyeen fyysiseen aktiivisuuteen (+20min/vrk p=0.002). Kuntoutusryhmien v\u00e4lill\u00e4 ei havaittu eroja kevyen fyysisen aktiivisuuden muutoksessa (p=0.160). Kevyen aktiivisuuden muutosta selitt\u00e4v\u00e4 malli selitti 47.1% kevyen aktiivisuuden muutoksen vaihtelusta ja sopi hyvin aineistoon (F(9,23)=4.171; p=0.003). Kevyen fyysisen aktiivisuuden kasvuun liittyi kuntoutujan alhaisempi ik\u00e4 (\u00df=-0.527, p=0.004), alhaisempi kevyen aktiivisuuden m\u00e4\u00e4r\u00e4 l\u00e4ht\u00f6tilanteessa (\u00df=-0.544, p=0.004) sek\u00e4 el\u00e4m\u00e4nlaadun heikkeneminen psyykkisell\u00e4 (\u00df=-0.401, p=0.016) ja sosiaalisella osa-alueella (\u00df=-0.422, p=0.014) kuntoutuksen aikana. Lis\u00e4ksi kevyt fyysinen aktiivisuus heikkeni, jos kuntoutujan liikuntamotivaatio suuntautui kuntoutuksen aikana eneneviss\u00e4 m\u00e4\u00e4rin liikunnasta saatavaan mielihyv\u00e4\u00e4n ja nautintoon (\u00df=-0.485, p=0.040). Kevyt fyysinen aktiivisuus kohentui henkil\u00f6ill\u00e4, joilla kuntoutuksen aikana el\u00e4m\u00e4nlaadun ymp\u00e4rist\u00f6tekij\u00f6iss\u00e4 tapahtui parannusta (\u00df=0.544, p=0.002) ja, jotka kuntoutuksen aikana kokivat liikuntamotivaationsa kumpuavan eneneviss\u00e4 m\u00e4\u00e4rin liikunnan merkityksellisyydest\u00e4 (\u00df=0.398, p=0.037). \n\nTutkielman tulokset her\u00e4tt\u00e4v\u00e4t kysymyksi\u00e4, joita tulee vahvistaa laadukkaalla suuremman otoksen pitkitt\u00e4istutkimuksella sek\u00e4 monimenetelm\u00e4tutkimuksella. Jatkossa olisi aiheellista tutkia, miksi aktiivisuuden lis\u00e4ys keskittyy kuntoutujilla vain kevyeen aktiivisuuteen. Vaikutuksen suuntaamiseksi raskaampiin aktiivisuustasoihin, tulisi selvitt\u00e4\u00e4 keinoja, joilla t\u00e4m\u00e4 vaikutus saataisiin aikaan. Parhaiten hy\u00f6tyv\u00e4n kohderyhm\u00e4n selvitt\u00e4mist\u00e4 tulisi suunnata kuntoutusryhm\u00e4n sijaan ik\u00e4- ja aktiivisuustasotekij\u00f6ihin. Et\u00e4teknologian vaikuttavuutta voidaan ilmeisesti tukea suuntaamalla ohjausta el\u00e4m\u00e4nlaadun ymp\u00e4rist\u00f6tekij\u00f6iden kohentamiseen. Et\u00e4teknologiaa sis\u00e4lt\u00e4v\u00e4ss\u00e4 kuntoutuksessa tulisi huomioida, ett\u00e4 et\u00e4teknologiaa sis\u00e4lt\u00e4v\u00e4 kuntoutus n\u00e4ytt\u00e4isi lis\u00e4v\u00e4\u00e4n ulkoista liikuntamotivaatiota, josta psykososiaalinen hyvinvointi voi k\u00e4rsi\u00e4. New innovations for activating population has been sought from distance technology. The aim of this study was to investigate the effects of technology-based rehabilitation on different intensity levels of physical activity (light, moderate, vigorous, total). Physical, psychosocial and demographic differences between cardiac, musculoskeletal and vocational rehabilitation groups were clarified at the baseline. In addition, the physical activity changes during rehabilitation were compared between rehabilitation groups. In order to improve the impact of technology-based rehabilitation, other physical and psychosocial variables that could explain the improvement of physical activity were observed. The research was part of the wider research project: The effectiveness and feasibility of ordinary and technology-based distance rehabilitation of finnish social insurance institution. \n \nThe study is prospective cohort research, where rehabilitees (N=36) received counceling and support by attending course meetings as well as through technology. The physical activity was measured by wristband activity tracker before and after six months rehabilitation. The statistical methods were T-test for paired samples, Wilcoxon Signed Rank, one-way variance analysis, Kruskall-Wallis, Khii square and linear regression. \n \nThere were differences between groups in the baseline. Musculoskeletal and cardiac rehabilitation groups differed from each other in terms of total activity (p=0.009), light activity (p=0.050) and gender distribution (p=0.018) in the baseline. The rehabilitees in vocational group experienced their social quality of life lower than rehabilitees in musculoskeletal group (p=0.049) and cardiac group (p=0.036). The study presents results that technology-based rehabilitation affects only to the light physical activity (+20min/d p=0.002). There were no differencies in the change of light physical activity between the rehabilitation groups (p=0.160). The explanatory model explained 47.1% of the variability of the change in light physical activity. The model was suitable for the material (F(9,23)=4.171, p=0.003). The weaker growth of light physical activity was associated with a higher age of the rehabilitee (\u00df =-0.527, p =0.004), higher levels of light activity at baseline (\u00df =-0.544, p =.004), and impairment of mental (\u00df = -0.401, p =0.016) and social quality of life (\u00df = -0.422, p = 0.014) during rehabilitation. In addition, light physical activity decreased on those rehabilitees who were increasingly motivated by the pleasure and enjoyment of exercise (\u00df = -0.485, p =0.040). Light physical activity improved in those who experienced an improvement in the environmental aspects of quality of life (\u00df =0.544, p =0.002) during their rehabilitation. There were improvements of light physical activity if the meaningfulness of exercise was increased as rehabilitee\u2019s source of exercise motivation (\u00df =0.398, p =0.037). \n \nThe results of this study raise new questions for future research to answer. The results should be strengthened by highquality longitudinal and multi-method reaserches with larger sample. In the future it should be considered why the increase in rehabilitees\u2019 physical activity focuses only for light activity. There should be research considering new methods to direct the effect of rehabilitation to harder intensities of physical activity, since those are not reached with technology-based rehabilitation. In determining the target group that benefits the most of technology-based rehabilitation, the research should be directed to the age and physical activity factors instead of rehabilitation group. The effectiveness of technology-based rehabilitation can be promoted by targeting help in environmental factors of rehabilitee's life. Technology-based rehabilitation appears to increase external motivation, which can harm psychosocial wellbeing.
