T\u00e4m\u00e4n poikkileikkausaineistoa\r\nhy\u00f6dynt\u00e4v\u00e4n tutkielman tarkoitus oli selvitt\u00e4\u00e4, onko 47 - 55 -vuotiaiden naisten vaihdevuosistatuksen\r\nja lihasmassan v\u00e4lill\u00e4 yhteytt\u00e4 sek\u00e4 onko vaihdevuosien, lihasmassan ja toimintakyky\u00e4\r\nmittaavien k\u00e4den puristusvoiman ja maksimaalisen k\u00e4velynopeuden v\u00e4lill\u00e4 yhteytt\u00e4. Tutkielman tarkoituksena\r\noli my\u00f6s selvitt\u00e4\u00e4 sarkopenian esiintyvyytt\u00e4 eri vaihdevuosiryhmiss\u00e4.\r\nTutkielman aineisto koostui Jyv\u00e4skyl\u00e4n yliopiston Gerontologian tutkimuskeskuksen ja Liikuntatieteellisen\r\ntiedekunnan Estrogeeni, vaihdevuodet ja toimintakyky (ERMA) -tutkimuksen alkumittausaineistosta.\r\nTutkittavat jaettiin verin\u00e4ytteest\u00e4 mitattujen hormonipitoisuuksien ja vuotop\u00e4iv\u00e4kirjan\r\nperusteella nelj\u00e4\u00e4n ryhm\u00e4\u00e4n: pre-menopaussi (n=237), aikainen perimenopaussi- (n=182), my\u00f6h\u00e4inen\r\nperimenopaussi- (n=193) ja postmenopaussiryhm\u00e4\u00e4n (n=290). Lihasmassa (kg) mitattiin DXAmittauksella.\r\nK\u00e4den puristusvoima (kg) mitattiin voimadynamometrill\u00e4 ja maksimaalinen k\u00e4velynopeus\r\n(m/s) 10 metrin matkalta. Vaihdevuosiryhmien v\u00e4lisi\u00e4 keskiarvoeroja tarkasteltiin yksisuuntaisella\r\nvarianssianalyysill\u00e4, c\r\n2\r\n-testill\u00e4, Kruskall-Wallisin testill\u00e4 ja kovarianssianalyysill\u00e4 (ANCOVA),\r\njossa kovariaatteina olivat fyysinen aktiivisuus ja rasvamassa.\r\nTutkielman tulosten mukaan vaihdevuosistatus oli yhteydess\u00e4 lihasmassaan (kg), lihasmassaindeksiin\r\n(kg/m2\r\n) ja k\u00e4den puristusvoimaan (p<0.001). Lihasmassassa pre-menopaussiryhm\u00e4n keskiarvo\r\n(43.0\u00b14.3kg) oli korkeampi kuin my\u00f6h\u00e4isen perimenopaussin (41.8\u00b14.5kg, p=0.002) ja postmenopaussiryhm\u00e4n\r\nkeskiarvot (41.5\u00b14.1kg, p<0.001). My\u00f6s lihasmassaindeksiss\u00e4 pre-menopaussiryhm\u00e4n\r\nkeskiarvo (6.7\u00b10.6 kg/m2\r\n) oli korkeampi kuin sek\u00e4 my\u00f6h\u00e4isen perimenopaussiryhm\u00e4n\r\n(6.6\u00b10.6 kg/m\r\n2\r\n, p=0.043) ett\u00e4 postmenopaussiryhm\u00e4n keskiarvo (6.5\u00b10.6 kg/m\r\n2\r\n, p=0.001). K\u00e4den\r\npuristusvoimassa pre-menopaussiryhm\u00e4n keskiarvo (31.3\u00b16.0kg) ja aikaisen perimenopaussiryhm\u00e4n\r\nkeskiarvo (32.2\u00b16.1kg) olivat korkeammat kuin postmenopaussiryhm\u00e4n keskiarvo (30.0\u00b15.4kg,\r\np<0.001). Pre-sarkopenian esiintyvyys oli pre-menopaussiryhm\u00e4ss\u00e4 2.9%, aikaisessa perimenopaussiryhm\u00e4ss\u00e4\r\n2.1%, my\u00f6h\u00e4isess\u00e4 perimenopaussiryhm\u00e4ss\u00e4 3.1% ja postmenopaussiryhm\u00e4ss\u00e4 5.8%.\r\nSarkopenian esiintyvyys oli aikaisessa - ja my\u00f6h\u00e4isess\u00e4 perimenopaussissa 0.5%.\r\nN\u00e4ytt\u00e4\u00e4 silt\u00e4, ett\u00e4 mit\u00e4 pidemm\u00e4lle vaihdevuodet etenev\u00e4t, sit\u00e4 v\u00e4hemm\u00e4n on lihasmassaa, mik\u00e4 edelleen\r\nn\u00e4ytt\u00e4\u00e4 olevan yhteydess\u00e4 my\u00f6s k\u00e4den puristusvoimaan. Tulosten perusteella sarkopenian esiintyvyys\r\neri vaihdevuosiryhmiss\u00e4 oli eritt\u00e4in alhainen, mutta menopaussi-ik\u00e4\u00e4 l\u00e4hestyvien naisten olisi\r\nkuitenkin viimeist\u00e4\u00e4n t\u00e4ss\u00e4 vaiheessa kiinnitett\u00e4v\u00e4 huomiota lihasmassaa yll\u00e4pit\u00e4viin elintapoihin.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "Aging declines muscle mass which is associated with impairments in physical performance. Menopause\r\nis a normal period in women\u2019s life and menopause related hormonal changes could cause decline\r\nin muscle mass already in middle-age. A cross-sectional data was used in this study. The aim\r\nwas to determine the association of menopause status with muscle mass and physical performance,\r\nas well as, to determine the prevalence of sarcopenia in different menopause status.\r\nThis study is based on the baseline data of Estrogenic Regulation of Muscle Apoptosis (ERMA)-\r\nstudy. Women aged from 47 to 55 years were assigned to pre (n=237)-, early peri (n=182)-, late peri\r\n(n=193) - and postmenopausal (n=290) groups based on their hormonal status and menstrual diary.\r\nDual-energy X-ray absorptiometry (DXA) was used to measure muscle mass (kg), a dynamometer to\r\nmeasure grip strength and a walking speed test of 10-meter distance to measure maximal gait speed\r\n(m/s). One way anova, c\r\n2 -test, Kruskal-Wallis test and analysis of covariance (ANCOVA) were used\r\nto delineate differences between menopausal groups. Used covariates were physical activity and fatmass.\r\nLinear regression models were constructed to estimate the association of gait speed and grip\r\nstrength with other variables.\r\nIn the present study, menopause status was significantly associated with muscle mass, muscle mass\r\nindex and grip strength (p<0.001). Significant group differences in muscle mass were between premenopausal\r\n(43.0\u00b14.3kg) and late perimenopausal (41.8\u00b14.5kg, p=0.002) and postmenopausal group\r\n(41.5\u00b14.1kg, p<0.001). The muscle mass index differed between the pre-menopausal (6.7\u00b10.6 kg/m2\r\n)\r\nand late perimenopausal (6.6\u00b10.6 kg/m2\r\n, p=0.043) and postmenopausal group (6.5\u00b10.6 kg/m2\r\n,\r\np=0.001). Also, the grip strength differed between pre-menopausal (31.3\u00b16.0kg) and postmenopausal\r\ngroup (30.0\u00b15.4kg, p<0.001). In addition, early perimenopausal group (32.2\u00b16.1kg) differed from\r\nthe postmenopausal group (p<0.001). In the present study, 2.9% of pre-menopausal, 2.1 % of early\r\nperimenopausal, 3.1 % of late perimenopausal and 5.8% of postmenopausal women were classified\r\nas having pre-sarcopenia. 0.5 % of early perimenopausal and 0.5 % of late perimenopausal women\r\nwere classified as having sarcopenia.\r\nIt seems that the further the menopausal stage progress, the less muscle mass, which still seems to\r\naffect the grip force.
