Aliravitsemuksen terveydelliset haitat tiedet\u00e4\u00e4n hyvin, mutta aliravitsemuksen ja i\u00e4kk\u00e4iden ihmisten el\u00e4m\u00e4nlaadun v\u00e4lisi\u00e4 yhteyksi\u00e4 on toistaiseksi tutkittu viel\u00e4 v\u00e4h\u00e4n. \nT\u00e4m\u00e4n tutkimuksen tavoitteena oli selvitt\u00e4\u00e4, onko ravitsemuksella yhteytt\u00e4 65\u201395-vuotiaiden kokemaan el\u00e4m\u00e4nlaatuun kotihoidossa. Tutkimus toteutettiin osana Keski-Suomen maakunnan vanhuspalvelujen kotihoidossa teht\u00e4v\u00e4\u00e4 tutkimusta.\nTutkimuksessa oli mukana kolmen keskisuomalaisen kunnan kotihoidon asiakkaita (n=69). Suurin osa tutkittavista (61 %) oli yli 80-vuotiaita ja naisten osuus koko joukosta oli 70 %. Ulkona heist\u00e4 liikkui 81 % joko apuv\u00e4lineen kanssa tai ilman. Suurimmalla osalla (84 %) kotihoidon palvelut olivat jatkuneet yli vuoden. \nTutkimus toteutettiin p\u00e4\u00e4osin kotihaastatteluna strukturoitua kyselylomaketta k\u00e4ytt\u00e4en. Ravitsemustilannetta arvioitiin Mini Nutritional Assessment (MNA)-mittarilla ja lis\u00e4kysymyksill\u00e4. Parempaa ravitsemustilannetta kuvaa pisteet 24 - 30 ja huonompaa 23.5 tai sit\u00e4 alhaisempi pistem\u00e4\u00e4r\u00e4. El\u00e4m\u00e4nlaatumittarina k\u00e4ytettiin Maailman terveysj\u00e4rjest\u00f6n el\u00e4m\u00e4nlaatumittarin lyhyemp\u00e4\u00e4 versiota (WHOQOL-BREF). Tilastollisina menetelmin\u00e4 k\u00e4ytettiin frekvenssijakaumien tarkastelua ja riippumattomien otosten t-testi\u00e4. \nRavitsemustilanteen ja el\u00e4m\u00e4nlaadun yhteys oli tilastollisesti merkitsev\u00e4 yleisen el\u00e4m\u00e4nlaadun kokemuksessa (P= 0.025). Paremmassa ravitsemustilanteessa olevat kokivat yleisen el\u00e4m\u00e4nlaatunsa paremmaksi kuin heikommassa ravitsemustilanteessa olevat. Samanlainen yhteys oli my\u00f6s el\u00e4m\u00e4nlaadun sosiaalisessa ulottuvuudessa (P=0.012). Paremmassa ravitsemustilanteessa olevat kokivat sosiaalisen el\u00e4m\u00e4nlaatunsa paremmaksi kuin heikommassa ravitsemustilanteessa olevat. Sen sijaan ravitsemuksen yhteydest\u00e4 el\u00e4m\u00e4nlaadun fyysiseen, psyykkiseen ja ymp\u00e4rist\u00f6-ulottuvuuteen ei saatu tilastollisesti merkitt\u00e4v\u00e4\u00e4 n\u00e4ytt\u00f6\u00e4, mutta tulokset olivat samansuuntaiset kuin sosiaalisessa ulottuvuudessa. Yhteiskunnallisesti tutkimuksen tuloksia voidaan hy\u00f6dynt\u00e4\u00e4 esimerkiksi vanhuspalveluissa ennaltaehk\u00e4isev\u00e4n ty\u00f6n suunnittelussa ja kehitt\u00e4misess\u00e4. T\u00e4ll\u00e4 voisi olla yhteiskunnan menoja hillitsev\u00e4 vaikutus niin palvelujen tarpeen kuin kustannusten kehityksenkin suhteen.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "The risk of malnutrition among home care clients is increasing because of the growth of average age and weakening health condition among elderly service users. We have a lot of information on the health disadvantages of malnutrition, but so far the links between malnutrition and the quality of life of older people have been studied to a very limited extent.\nThe aim of this study was to find out whether nutrition is related to the quality of life experienced by 65-95-year-olds in home care clients. The study was carried out as a part of larger research for home care services in Central Finland.\nThe study included clients of home care from three Central Finland municipalities (n = 69). Most of the examinees (61%) were over the age of 80 and women accounted for 70% of the total. 81% of them could move out with or without mobility aids. Most of them (84%) the home care services had lasted for more than a one year.\nData collection was mainly performed as home interviews using a structured questionnaire. The nutrition status was evaluated with Mini Nutritional Assessment (MNA) meter and open questions. A better nutritional situation is represented by score 24 to 30 and worse score of 23.5 or lower. The short version of World Health Organization\u2019s quality of life assessment WHOQOL-BREF was utilized. Statistical methods included assessment frequency distributions and t-test for independent samples. \nThe nutrition status was statistically significantly associated with the quality of life (P = 0.025). Participants with better nutrition status experienced better overall quality of life than participants with worse nutrition status. The nutrition status was also statistically significantly associated with the social dimension of the quality of life (P = 0.012). Participants with better nutrition status experienced better social quality of life than participants with worse nutrition status. On the other hand, the relation of nutrition on the physical, mental and environmental dimensions of quality of life was not statistically significant, although the results tended to be similar in all dimensions. Results of this can be used in designing and developing the preventative work in elderly services.
