Tutkimuksessa tarkasteltiin neuvonantamisen toteutumista opiskeluterveydenhuollossa neljän eri teeman osalta: uni ja nukkuminen, ravitsemus, liikkuminen ja päihteet. Terveydenhoitajien ja opiskelijoiden välisen vuorovaikutuksen ja neuvonannon rakentumista tarkasteltiin erityisesti kysymys – vastausvierusparien kautta. Tutkimusaineisto kerättiin tammikuussa 2017. Tutkimukseen osallistui viisi ammatillisen oppilaitoksen terveydenhoitajaa ja viisi 16–20-vuotiasta opiskelijaa. Terveysneuvontatilanteet tallennettiin videokameralla ja sanelukoneella. Tutkimusaineisto analysoitiin soveltaen keskustelunanalyysin menetelmällä aineistolähtöisesti. Neuvonanto toteutui pääasiassa hoitajan arvioiman muutostarpeen pohjalta. Hoitajien antamat neuvot olivat suoria tai epäsuoria ohjeita toimintatavan muuttamiseksi tai ongelman ratkaisemiseksi. Neuvoja annettiin myös kysymyksiin sidottuna. Neuvoissa terveydenhoitajat kertoivat, mitä opiskelijan tulisi tehdä tai olla tekemättä. Neuvoja annettiin tyypillisesti teeman loppupään episodeissa, mutta myös yksittäisissä episodeissa keskustelun eri vaiheissa. Neuvot annettiin tyypillisimmin joko käyttämällä yksikön 2.persoonamuotoa tai passiivia. Opiskelijat vastaanottivat neuvot joko hyväksyen tai hyläten. Aina opiskelijoille ei jäänyt tilaa kommentoida annettua neuvoa. Terveydenhoitajat hallitsivat keskustelujen kulkua kysymyksillä. Terveydenhoitajien esittämien suljettujen kysymysten määrä oli suurempi kuin avoimien kysymysten määrä. Opiskelijoiden vastaukset hoitajien esittämiin kysymyksiin muodostuivat puheenvuoron aikana joko yhdestä sanasta, lauseesta tai useamman lauseen kokonaisuudesta. Useamman lauseen kokonaisuuksissa lauseet selittivät tai tarkensivat opiskelijan edellä antamaa vastausta. Terveydenhoitajat reagoivat opiskelijoiden vastauspuheenvuoroihin joko dialogipartikkeleilla, kysymyslauseilla, palautteella, mielipiteen tai arvion ilmauksella, ehdotuksella tai neuvonnalla. Mikäli terveydenhoitaja reagoi opiskelijan puheenvuoroon vain yksittäisellä dialogipartikkelilla, jatkoi opiskelija useimmiten puhetta. Dialogipartikkelin jatkoksi esitetty kysymys puolestaan rajasi opiskelijan puhetta. Keskusteluaiheista toiseen siirryttiin pääasiassa terveydenhoitajan esittämän kysymyksen kautta. Tulosten pohjalta voidaan todeta, että terveydenhoitajat olivat enemmän hoitaja- kuin opiskelijalähtöisiä ja antoivat enemmän suoria neuvoja kuin ohjausta. Jotta asiakaslähtöisyyttä, opiskelijoiden muutosvalmiutta sekä aktiivisuutta vuorovaikutustilanteissa voitaisiin lisätä, on hoitajien osaamista vaikuttavan neuvonnan osalta kuitenkin tarpeen kehittää. Lisäksi terveydenhoitajat tarvitsevat enemmän tietoa teoriapohjaisesta ja näyttöön perustuvasta neuvonnasta. Asiasanat: terveydenhoitajat, nuoret, neuvonta, vuorovaikutus Arja Taipale (2018). Advice given by public health nurses during the health dialogue in vocational schools. Faculty of Sport and Health Sciences, University of Jyväskylä, Master’s thesis, 83 pages, 13 appendices. The health counseling is realized by means of interactions. In this thesis, giving advice was observed on four different themes: sleep, nutrition, exercising and substance use. The development of the interaction between public health nurses and students and counseling conversation was observed especially based on question-answer pairs. The research material was collected in January 2017. Five public health nurses and five students aged between 16 and 20 from vocational schools participated in this research. The health counseling sessions were recorded with a video camera and a digital recorder. The research material was analyzed by applying the conversation analysis method. Advice was mainly given based on the need for change evaluated by the nurse. The advice given by the public health nurses consisted of direct or indirect instructions to change the way of acting or to solve the problem. Advice was also given as a part of the questions. When offering advice, the public health nurses told the student what they should do and should not do. The advice was typically given in episodes which were the last part of the theme, but some advice was given also in the different phases of the discussion in a single episode. The advice was given either using the second-person in singular or passive. The students either accepted or declined the advice. The students did not always get space to comment on the advice. The public health nurses lead the discussion by using the questions. The number of close-ended questions was greater than the number of open-ended questions among the questions the public health nurses asked the students. The students answered the questions made by the public health nurses using one word, one sentence or multiple sentences. In the case of multiple sentences, the student explained or expanded the answer given earlier. The public health nurses responded to students’ answers with a dialog particle, asking questions, giving feedback, expressing an opinion or an evaluation, giving a suggestion or advice. If the public health nurse responded with only one dialog particle, the student continued the discussion in most cases. The question asked after the dialog particle, on the other hand, limited the student’s speech. The transition from one topic to another mainly happened through the questions made by the public health nurse. Based on the results, counseling by public health nurses was more nurse-oriented than student-oriented and more direct than guiding. To increase the student-oriented approach, students’ readiness for change and students’ activity in the interaction situations, the public health nurses’ knowledge on influential counseling need to be improved.
