Tässä tutkielmassa selvitettiin lasten kardiorespiratorisen kunnon yhteyksiä plasman alaniiniaminotransferaasi- (ALT) ja gammaglutamyylitransferaasipitoisuuksiin (GGT). Menetelmät: Tutkimukseen osallistui 465 6-9-vuotiasta lasta. Kardiorespiratorinen kunto arvioitiin maksimaalisella polkupyöräergometritestillä maksimaalisena työmääränä (Wmax), ja se suhteutettiin erikseen kehon massaan (BM) sekä rasvattomaan massaan (LM). Kehon rasvaprosentti (BF%) määritettiin kaksienergisellä röntgenabsorptiometrialla (DXA). ALT- ja GGT-pitoisuudet määritettiin 12 tunnin paaston jälkeen otetusta verinäytteestä. Analyysimenetelmänä käytettiin iällä ja sukupuolella vakioitua lineaarista regressioanalyysiä. Tulokset: Kardiorespiratorinen kunto, sekä kehon massaan että rasvattomaan massaan suhteutettuna, oli käänteisesti yhteydessä GGT-pitoisuuksiin pojilla (Wmax/BM: B=–0,273; 95% CI=–0,397-(–0,150), P<0,001, R2adj=0,077 ja Wmax/LM: B=–0,180; 95% CI=–0,308-(–0,052), P=0,006, R2adj=0,034). Wmax/LM ja GGT:n yhteys säilyi pojilla merkitsevänä myös rasvaprosentilla vakioimisen jälkeen (B=–0,135; 95% CI=–0,262-(–0,009), P=0,036, R2adj=0,093). Kardiorespiratorinen kunto ei ollut yhteydessä ALT-pitoisuuksiin pojilla eikä ALT- tai GGT-pitoisuuksiin tytöillä. Rasvaprosentti oli yhteydessä ALT- ja GGT-arvoihin pojilla ja tytöillä kardiorespiratorisesta kunnosta riippumatta (P<0,022). Pohdinta: Tulosten perusteella parempi kardiorespiratorinen kunto oli käänteisesti yhteydessä GGT-pitoisuuksiin pojilla. Nämä tulokset viittaavat siihen, että pojilla matalan kestävyyskunnon omaavilla pojilla saattaa olla kohonnut NAFLD:n riski. Tuloksia pitää kuitenkin tulkita varoen, sillä maksaentsyymipitoisuudet eivät ole suoria maksan rasvapitoisuuden mittareita. Siitä huolimatta havainnot korostavat riittävän kestävyyskunnon merkitystä terveydelle, sekä kestävyys-kunnon seulonnan ja alhaisen kardiorespiratorisen kunnon parantamisen tärkeyttä lapsilla kardiometabolisen terveyden edistämiseksi. Background and purpose: Non-alcoholic fatty liver disease (NAFLD) is a risk factor for type 2 diabetes and cardiovascular diseases in adults. Although cardiorespiratory fitness (CRF) has been inversely associated with cardiometabolic risk in children, there are only few previous studies on the associations of CRF with the indicators of NAFLD in paediatric populations. Therefore, I examined the association of CRF with plasma alanine aminotransferase (ALT) and gamma-glutamyltransferase (GGT). Methods: Altogether 465 6–9-year-old children participated in the study. CRF was assessed by maximal cycle ergometer test and defined as maximal workload (Wmax) normalised for body mass (BM) and lean mass (LM). Body fat percentage (BF%) was assessed by dual-energy x-ray absorptiometry (DXA). ALT and GGT were assessed from venous blood samples after 12h fast. The data were analysed using linear regression analyses adjusted for age and sex. Results: CRF, scaled with BM or LM, was inversely associated with GGT concentration in boys (Wmax/BM: B=-0.273, 95% CI=-0.397 to -0.150, P<0.001, R2adj=0.077 and Wmax/LM: B=-0.180, 95% CI=-0.308 to -0.052, P=0.006, R2adj=0.034). The association between Wmax/LM with GGT in boys remained significant after further adjustment for BF% (B=-0.135, 95% CI=-0.262 to -0.009, P=0.036, R2adj=0.093). CRF was not associated with ALT in boys and ALT or GGT in girls. BF% was directly related to ALT and GGT in boys and girls independent of CRF (p<0.022). Conclusions: The present results showed that CRF was inversely associated with lower GGT concentration in boys. These results suggest that boys with lower CRF may be at increased risk of NAFLD already in childhood. However, these results should be interpreted cautiously as liver enzymes are not direct indicators of liver fat. Nevertheless, these observations highlight the im-portance of screening and support interventional approach with children with low CRF in order to improve their cardiometabolic health.
