Fyysisell\u00e4 aktiivisuudella on my\u00f6s osoitettu olevan yhteys v\u00e4hentyneisiin masennusoireisiin. T\u00e4m\u00e4n pro gradu-tutkielman tarkoituksena oli selvitt\u00e4\u00e4 fyysisen aktiivisuuden, masennusoireiden ja tyypin 2 diabeteksen v\u00e4list\u00e4 yhteytt\u00e4 sek\u00e4 kartoittaa sosiodemografisten ja kliinisten tekij\u00f6iden yhteytt\u00e4 tyypin 2 diabetekseen.\r\nPoikkileikkaustutkimukseen kutsuttiin yhteens\u00e4 4500 tutkittavaa, joista 2745 i\u00e4lt\u00e4\u00e4n 45-74 \u2013vuotiasta osallistui mukaan D2D-masennustutkimukseen. Tutkittavat rekrytoitiin kolmen eri sairaanhoitopiirin alueelta: Keski-Suomesta, Pirkanmaalta ja Etel\u00e4-Pohjanmaalta. Tutkittaville suoritettiin terveystarkastus, jonka aikana he t\u00e4yttiv\u00e4t kyselylomakkeita. Tutkittavilla todettiin olevan tyypin 2 diabetes, jos sairaus oli itse raportoitu ja l\u00e4\u00e4k\u00e4rin aikaisemmin toteama tai terveystarkastuksen yhteydess\u00e4 todettu. Masennusoireita kartoitettiin Beckin kyselylomakkeella ja fyysist\u00e4 aktiivisuutta International Physical Activity Questionnaire (IPAQ) kyselylomakkeen lyhyell\u00e4 versiolla. Fyysisen aktiivisuuden, masennusoireiden ja tyypin 2 diabeteksen v\u00e4list\u00e4 yhteytt\u00e4 kartoitettiin Pearson Chi-Square testill\u00e4 ja sosiodemografisten ja kliinisten muuttujien yhteytt\u00e4 tyypin 2 diabetekseen tarkasteltiin logistisella regressioanalyysilla. \r\nTyypin 2 diabeteksen esiintyvyys oli 23 % alhaisessa, 17 % kohtalaisessa ja 13% korkeassa fyysisen aktiivisuuden luokassa (p<0.001). My\u00f6s masennusoireiden esiintyvyys aleni fyysisen aktiivisuuden kasvaessa. Kohtalaisesti liikkuvilla tyypin 2 diabeteksen esiintyvyys oli suurempaa masennusoireisilla henkil\u00f6ill\u00e4 verrattaessa niihin, joilla ei ollut masennusoireita (p=0.038). Logistisen regressioanalyysin perusteella korkea ik\u00e4, rasva-aineenvaihduntah\u00e4iri\u00f6t, korkea verenpaine, suuri vy\u00f6t\u00e4r\u00f6nymp\u00e4rys ja alhainen fyysinen aktiivisuus oli yhteydess\u00e4 korkeampaan riskiin sairastua tyypin 2 diabetekseen, kun taas naimisissa olo tai avoliitto, koulutusvuodet, tupakointi, painoindeksi, krooniset syd\u00e4nsairaudet tai masennusoireet eiv\u00e4t osoittaneet yhteytt\u00e4 tyypin 2 diabetekseen. \r\nAlhainen fyysinen aktiivisuus on yhteydess\u00e4 korkeampaan masennusoireiden ja tyypin 2 diabeteksen esiintyvyyteen. Masennusoireet eiv\u00e4t n\u00e4ytt\u00e4isi olevan kuitenkaan yhteydess\u00e4 suurentuneeseen riskiin sairastua tyypin 2 diabetekseen, kun kardiometaboliset riskitekij\u00e4t ja alhainen fyysinen aktiivisuus otettiin analyysissa huomioon, joten pitkitt\u00e4isseurantatutkimuksia tarvitaan asian selvitt\u00e4miseen. T\u00e4m\u00e4n tutkimuksen tulosten perusteella voidaan kuitenkin todeta, ett\u00e4 aikaisen vaiheen terveysk\u00e4ytt\u00e4ytymisen sek\u00e4 metabolisten riskitekij\u00f6iden seulominen terveydenhuollossa on t\u00e4rke\u00e4\u00e4 tyypin 2 diabeteksen sek\u00e4 masennuksen ehk\u00e4isyss\u00e4.", "language": "fi", "element": "description", "qualifier": "abstract", "schema": "dc"}, {"key": "dc.description.abstract", "value": "Physical activity is a corner stone of type 2 diabetes treatment and prevention. The association between physical activity and lower prevalence of depressive symptoms has been well documented. The aim of this study was to investigate associations between physical activity, depressive symptoms and type 2 diabetes and furthermore associations between sociodemographic and clinical variables in type 2 diabetes. \r\nOut of 4500, 2 745 participants aged 45-74 from three hospital districts of central Finland, Pirkanmaa and southern Ostrobothnia attended in this cross-sectional study. All participants went through a health examination and sociodemographic variables were assessed with a questionnaire. The participants were defined as having type 2 diabetes if it was self-reported and diagnosed by a doctor or screened during the health examination. Depressive symptoms were determined with the 21-item Beck Depression Inventory (BDI-21) with cutoff score \u226510 and physical activity with International Physical Activity Questionnaire (IPAQ). The association between physical activity, depressive symptoms and type 2 diabetes were tested by Pearson Chi-Square test and logistic regression models were used to examine the factors predicting type 2 diabetes.\r\nThe prevalence of type 2 diabetes was 23 % in low, 17 % in moderate and 13 % in high level PA groups (p <0.001). The prevalence of depressive symptoms decreased with increasing physical activity. The prevalence of type 2 diabetes were higher in people with depressive symptoms compared to those without depressive symptom in moderate physical activity level (p=0.038). Logistic regression models showed that higher age and high blood pressure, dyslipidemia, waist circumference and low physical activity were significantly related to a higher risk of type 2 diabetes; whereas married or common law married, education in years, current smoking, BMI, chronic heart disease and depressive symptoms showed no association with type 2 diabetes. \r\nLow physical activity is associated with higher prevalence of depressive symptoms and type 2 diabetes. Depressive symptoms did not seem to increase the risk of having type 2 diabetes after adjusting with all potential confounders. More longitudinal studies are needed to clarify the association.
