Biomekaanisia mekanismejä, jotka ovat vastuussa pohjallisten kliinisestä tehokkuudesta, ei vielä täysin ymmärretä. Useat tutkimukset ovat raportoineet pohjallisten akuutteja vaikutuksia juoksun kinematiikkaan, kinetiikkaan sekä lihasaktiivisuuksiin. Tämän tutkimuksen tarkoituksena oli selvittää pohjallisintervention mahdollisia pitkäaikaisia vaikutuksia juoksun kinematiikkaan sekä kinetiikkaan. Lisäksi tavoitteena oli selvittää pohjallisten vaikutusta submaksimaalisen juoksun taloudellisuuteen. Tutkimukseen osallistui 13 naispuolista harrastejuoksijaa (27 ± 6.2 vuotta). Kaikilla osallistujilla todettiin nilkan ylipronaatio. Dynaaminen juoksun liikeanalyysi (4.0 ± 0.3 m/s) tehtiin sekä ennen että jälkeen 4-viikon mittaisen pohjallisintervention. Data kerättiin käyttäen kahdeksan infrapunakameran ja viiden voimalevyn järjestelmää. Jokaiselle koehenkilölle tehtiin yksilöllisesti mediaalisesti muotoiltu pohjallinen (MMP). Juoksun taloudellisuus mitattiin 200-m pituisella sisäjuoksuradalla käyttäen kannettavaa happikaasuanalysaattoria. Pohjallisinterventio vähensi kantaluun huippueversiota sekä MMP:llä (-4.7°, p<0.05) että normaaleilla pohjallisilla (-4.4°, p<0.05). Lisäksi sääriluun rotaation liikelaajuus vähentyi suhteessa kantaluuhun sekä MMP:llä (-4.8°, p<0.05) että normaaleilla pohjallisilla (-5.6°, p<0.05). Suurimmat erot pohjallisten välillä havaittiin päkiä- ja kantaluusegmenttien välisessä liikkeessä. MMP vähensi merkittävästi päkiän huippupronaatiota suhteessa kantaluuhun (- 1.9°, p<0.05) sekä kasvatti jalkaholvin kaaren suuruutta koko askeleen ajalta. MMP ei kuitenkaan merkittävästi vähentänyt näiden segmenttien välistä liikettä. Yhdessäkään kineettisessä frontaali- tai transversaalitason muuttujassa ei ollut merkittäviä muutoksia mittauskertojen tai pohjallisten välillä. Submaksimaalisen juoksun hapenkulutuksessa ei ollut eroa pohjallisten välillä. Tulosten perusteella MMP-interventio vähentää merkittävästi kantaluun huippueversiota ja sääriluun rotaation liikelaajuutta. Lisäksi pohjallisintereventiolla näyttäisi olevan siirrännäisvaikutus myös normaaleihin pohjallisiin. Tässä tutkimuksessa käytettyjä mediaalisesti muotoiltuja pohjallisia voidaan käyttää osana kliinistä protokollaa erinäisten alaraajavammojen hoidossa, ilman negatiivisia vaikuksia juoksun taloudellisuuteen. Pronation of the foot is naturally occurring movement during human locomotion. However, excessive pronation is potentially harmful and is associated with the development of running related lower limb injuries. Methods to treat and prevent these injuries include foot orthoses, which have produced positive results. The biomechanical mechanisms responsible for the clinical efficacy of the foot orthoses are not well known, but number of studies has reported their acute effects on lower extremity kinematics, kinetics and muscle activities during running. The aim of this research was to study the possible long-term effects of medially posted insole (MPI) intervention on running kinematics and kinetics. The second aim was to investigate the effects of MPI’s on running economy in subjects with overpronation. Thirteen female recreational runners with overpronation were recruited for this study (27 ± 6.2 years). Dynamic running (4.0 ± 0.3 m/s) analysis was conducted before and after a 4-week MPI intervention. Data was collected using an eight-camera system and five force platforms. Running economy analysis was conducted after the 4-week MPI intervention using a portable telemetric !! analyzer at a 200 m indoor running track. MPI intervention decreased peak rearfoot eversion by 4.7° and 4.4°, for MPI (p<0.05) and normal insole (p<0.05), respectively. The results also showed a significant reduction in tibial rotation range of motion in respect to rearfoot, for both MPI (-4.8°, p<0.05) and normal insole (-5.6°, p<0.05). Differences in insole condition were mostly between forefoot and rearfoot segments. MPI’s significantly decreased peak forefoot pronation in relation to rearfoot (-1.9°, p<0.05) and increased forefoot-rearfoot dorsiflexion throughout the stance phase. However, MPI’s did not significantly reduce motion between the two segments. No significant effects were found for any frontal or transversal plane kinetic variables between sessions or between insole conditions. There were no changes in the oxygen cost of submaximal running with MPI when compared to normal insoles. Current results suggest that there may be an accommodation period, which is needed to see the full effects of MPI on foot kinematics. There may also be a “carry-over” effect, in which similar kinematic changes can be seen even with normal insoles after the intervention. MPI’s used in the current study can be used as a part of clinical protocol to manage various lower extremity injuries without negatively affecting running economy.
