Suomessa tehdään vuosittain noin 7500 lonkan tekonivelleikkausta ja väestön ikääntyessä leikkausten määrä tulee todennäköisesti lisääntymään. Samaan aikaa hoitoajat ovat lyhentyneet, mikä lisää tarvetta fysioterapiaohjauksen kehittämiseen. Tekonivelleikkauksesta kuntoutuminen voidaan nähdä yksilöllisenä kasvu- ja oppimisprosessina, mutta aihetta on aiemmin tutkittu vähän fysioterapian alueella. Tämän pro gradu -tutkimuksen tarkoituksena on ymmärtää lonkan tekonivelleikkauksesta kuntoutumista oppimisen näkökulmasta. Tutkimus on osa Jyväskylän yliopiston hallinnoimaa Potilasohjaus fysioterapiassa –tutkimushanketta. Tutkimustuloksia voidaan hyödyntää lonkan tekonivelleikkauspotilaiden fysioterapiaohjauksen kehittämisessä. Tutkimuksen kohderyhmänä ovat aikuiset lonkan tekonivelleikkauksen läpikäyneet potilaat. Tutkimusaineisto on kerätty alkuvuodesta 2010. Tutkimusaineiston muodostavat 10 lonkan tekonivelleikkauspotilaan (ka 69,7 v) haastattelut. Potilaita on haastateltu kaksi kertaa; n. viikko ennen leikkausta ja 3-5 viikkoa leikkauksen jälkeen. Haastattelumenetelmänä on käytetty teemahaastattelua, jota on syvennetty vapaaseen kerrontaan kannustavilla lisäkysymyksillä. Aineisto analysoitiin aineistolähtöisesti narratiivisella analyysimenetelmällä. Analyysin tuloksena syntyi neljä oppimisen tarinamallia: ongelman ratkaisijoiden, ennakkoon valmistautuvien, ohjausta tarvitsevien ja järjestelmään luottavien tarinamallit. Oppimisen tarinamalleista nousi esiin erilaisia oppimisen teorioita perustuen kuntoutujien toimintaan ja oppimiseen kuntoutumisprosessin aikana. Tarinamalleista on löydettävissä yhtymäkohtia kokemuksellisen oppimisen, ongelmaperustaisen oppimisen, kognitiivisen oppimisen, sosiaalisen oppimisen ja ei-oppimisen teorioihin. Tarinamallit ilmentävät myös kuntoutujien toimijuuden astetta kuntoutumisessaan. Tarinamallit osoittavat tarpeen pohtia, kuinka kuntoutuksessa ja fysioterapeuttisessa ohjauksessa voidaan huomioida kuntoutujien eri oppimistyylit sekä tukea heidän aktiivista toimijuuttaan. With the ageing population the operation rate is likely to increase. At the same time the period during which the patient receives care has shortened which has increased the need for physiotherapy counselling. Recovering from total hip arthroplasty can be seen as an individual process of growth and learning which is a subject that has previously been studied very little in the field of physiotherapy. The aim of this master’s thesis is to understand recovering from hip arthroplasty from the standpoint of learning. This master’s thesis is a part of a research project called ‘Patient counselling in Physiotherapy’ conducted by University Of Jyväskylä. The results of the research project will benefit the development of physiotherapy counselling of total hip arthroplasty patients. The target group of this master’s thesis is made up of adult patients that have undergone total hip arthroplasty. The raw data for the research has been collected at the beginning of the year 2010. The research material comprises interviews of 10 total hip arthroplasty patients in the average age of 69,7 years. The patients have been interviewed approximately a week before operation and again 3 to 5 weeks after the operation. The interview method has been theme interviews that have been deepened with additional questions to encourage the interviewee to free narration. The research material was analyzed with a narrative method of analysis. The analysis produced 4 story models for learning: problem solvers’ story model, the story model of those prepared in advance, the story model of those in need of counselling and the story model of those who trust the system. The story models of learning produced different theories of learning based on the patients’ activity and learning during the process of rehabilitation. In the story models can be found connections to theories of experiential learning, problem-based learning, cognitive learning, social learning and non-learning. The story models express the patients’ degree of agency in rehabilitation. The story models also show a need to deliberate how to take patients’ different styles of learning into account and how to support their active agency.
