Sairaalassa tapahtuvan fysioterapeuttisen ohjauksen haasteena on juurikin tilanteen kertaluonteisuus ja lyhytaikaisuus. Aikaisemmat tutkimukset ovat osoittaneet, ett\u00e4 asiakkaat ovat olleet tyytym\u00e4tt\u00f6m\u00e4mpi\u00e4 lyhyisiin fysioterapiakontakteihin verrattuna pidempiin fysioterapian hoitojaksoihin. Fysioterapeuttisen ohjauksen puhetavoista saatu tieto auttaa toteuttamaan ohjausta potilasl\u00e4ht\u00f6isemmin. Fysioterapeuttinen ohjaus j\u00e4sentyy t\u00e4m\u00e4n hetkisen tiedon mukaan tautien, kuntoutujan, vuorovaikutuksen ja fysioterapiaprosessin kautta. T\u00e4ss\u00e4 tutkimuksessa ohjausta tarkastellaan vuorovaikutuksen n\u00e4k\u00f6kulmasta. Onnistunut vuorovaikutus ja kommunikointi ovat sek\u00e4 onnistuneen hoidon ett\u00e4 potilasl\u00e4ht\u00f6isyyden edellytys. \r\nT\u00e4m\u00e4n tutkimuksen tarkoituksena on kehitt\u00e4\u00e4 sairaalavaiheessa tapahtuvaa fysioterapeuttista ohjausta tukemaan paremmin potilaan yksil\u00f6llist\u00e4 tilannetta, tarvetta sek\u00e4 kuntoutumista. Tutkimuksessa tarkastellaan fysioterapeuttisen ohjauksen puhetapoja sek\u00e4 ohjaustilanteessa rakentuvia merkityksi\u00e4 potilaan tilanteiseen aktiivisuuteen, potilasl\u00e4ht\u00f6isyyteen, leikkauksen j\u00e4lkeiseen arkeen ja tulevaisuuteen. T\u00e4m\u00e4 pro gradu tutkielma on osa laajempaa potilasohjaus tutkimusta, jota toteutetaan yhteisty\u00f6ss\u00e4 Jyv\u00e4skyl\u00e4n yliopiston ja Keski-Suomen keskussairaalan kanssa. Tutkimusyhteisty\u00f6n tavoitteena on saada tietoa potilaiden toimintakyky\u00e4 edist\u00e4v\u00e4st\u00e4 ohjuksesta leikkausten yhteydess\u00e4, sek\u00e4 kehitt\u00e4\u00e4 ohjausta kotona selviytymist\u00e4 edist\u00e4v\u00e4ksi.\r\nTutkimuksen aineisto oli kahdeksan sairaalassa kuvanauhoitettua fysioterapeuttista ohjaustilannetta. Tutkittavia potilaita oli seitsem\u00e4n ja fysioterapeutteja kuusi. Tutkimuksen aineisto analysoitiin diskurssianalyysi\u00e4 apuna k\u00e4ytt\u00e4en. Analyysiss\u00e4 mukailtiin diskurssianalyysin kolmion kulmakivi\u00e4; kommunikointi, konteksti ja merkitykset. Tutkimuksessa selvitettiin merkitysten rakentumista ohjaustilanteessa. \r\nAineistossa esiintyi kuusi erilaista fysioterapeuttisen ohjauksen puhetapaa: fysioterapeutin valtapuhe, selitt\u00e4v\u00e4 ohjauspuhe, sanottava ohjauspuhe, vuoropuheinen ohjauspuhe, potilasta aktivoiva ohjauspuhe ja kannustava ohjauspuhe. Potilaan tilanteinen aktiivisuus oli suurinta potilasta aktivoivan ohjauspuheen yhteydess\u00e4. Vuoropuheinen ohjauspuhe eteni ohjaustilanteessa fysioterapeutin ja potilaan v\u00e4lisen\u00e4 keskusteluna, johon molemmat osallistuivat tasavertaisesti. Selitt\u00e4v\u00e4ss\u00e4 ja sanottavassa ohjauspuheessa fysioterapeutti oli potilasta aktiivisempi ja fysioterapeutin valtapuheessa fysioterapeutin rooli oli hallitseva. Potilasl\u00e4ht\u00f6isyys toteutui parhaiten vuoropuheisen- sek\u00e4 potilasta aktivoivan ohjauspuheen yhteydess\u00e4. Leikkauksen j\u00e4lkeinen arki oli esill\u00e4 ohjauspuheissa p\u00e4\u00e4asiassa harjoitteiden ja opasvihkon yhteydess\u00e4. Vuoropuheisessa ja potilasta aktivoivassa ohjauspuheessa leikkauksen j\u00e4lkeinen arki sai my\u00f6s laajempia merkityksi\u00e4. Ainoastaan vuoropuheisen ohjauspuheen yhteydess\u00e4 luotiin n\u00e4kymi\u00e4 my\u00f6s pidemm\u00e4lle potilaan tulevaisuuteen. Sanottava ohjauspuhe on fysioterapialle erityist\u00e4, koska ohjauksen merkitykset kohdentuivat liikkeen ja harjoitteen laatuun. Selitt\u00e4v\u00e4n ohjauspuheen yhteydess\u00e4 merkitykset rakentuivat p\u00e4\u00e4asiassa opasvihkon yhteyteen. Kannustavaa ohjauspuhetta ilmeni kaikkien puhetapojen yhteydess\u00e4, kuitenkin siten, ett\u00e4 potilasta aktivoivassa ja vuoropuheisessa ohjauspuheessa se oli kiinte\u00e4 osa ohjausta. The aim of this study is to develop good physiotherapy practice in guidance between physiotherapist and patient. As the result of this study different ways to communicate in physiotherapy situations came out. The interest was also to study how those different ways to communicate appear in the aspect of patient\u00b4s activity after hip surgery at home and the patient centered point of view in the physiotherapy situation.\r\nPresent studies have shown that guidance in physiotherapy is been structured by the diseases, client, interaction and physiotherapy process. In this study the interest is in interaction. Successfull communication and interaction guarantee the good quality of care and patient centeredness. Physiotherapy situation after hip surgery is very short and happens only once or twice, so the challenges of the guidance are great. Earlier studies have shown that patients are most displeased in short term physiotherapy, so development of those situations are needed. This study is a part of the co-operation between Univeristy of Jyv\u00e4skyl\u00e4 and the Central hospital of Middle Finland. Aim of the co-operation is to develop patient education that activates the patient and advance the patient\u00b4s functional capacity after surgery. Also the aim is to support patient\u00b4s survival at home after surgery by patient education. \r\nMETHODS: This study included eight physiotherapy sessions with patient education in hospital after hip surgery. Physiotherapy sessions were videotaped. The target group of the study, were seven patients and six physiotherapists. All patients got through the hip surgery for the first time. The method of the analysis was discourse analysis. The interest in the analysis was the connection between the communication, context and different meanings that were constructed during the patient education situation in physiotherapy.\r\nRESULT: In this study there were six ways to communicate in the physiotherapy situations with patient education and guidance: power speech of physiotherapist, explanatory way of communicate, dialogue, empowerment speech, supportive way to communicate and guidance of quality of movement.\r\nThe supportance of patient\u00b4s postoperative condition was seen most in dialogue and in empowerment speech. Same ways of communicate supported also the patient\u00b4s activity in physiotherapy situations. When the aim of the guidance was the quality of exercise or quality of movement physiotherapists used lyricist way to communicate. This way of communicate was often supported by manual or visual guidance. It seemed that this way of communicate is speciality of physiotherapy. The power speech of physiotherapist was less supportive and it didn\u00b4t include patient centeredness or activity of the patient or survival at home after surgery. \r\nCONCLUSION: The evidence of this study shows that way to communicate in physiotherapy session should be developed similar to dialogue and empowerment speech. That way patient centeredness and supportance of patient\u00b4s postoperative condition at home are best noticed. It is also notable that guidance of movement quality is something that is unique to physiotherapy and it concentrates to movement quality.
