T\u00e4m\u00e4n Pro Gradu -tutkielman tarkoituksena on selvitt\u00e4\u00e4 kest\u00e4vyysliikunnan vaikutusta unen laatuun ja m\u00e4\u00e4r\u00e4\u00e4n keski-ik\u00e4isill\u00e4 unettomuudesta k\u00e4rsivill\u00e4 miehill\u00e4.\r\n\r\nSatunnaistetussa kontrolloidussa tutkimuksessa 42 v\u00e4h\u00e4n liikkuvaa vapaaehtoista 30\u201365-vuotiasta unettomuudesta k\u00e4rsiv\u00e4\u00e4 miest\u00e4 Keski-Suomen sairaanhoitopiirist\u00e4 (keski-ik\u00e4 51 [1.6] vuotta) jaettiin liikunta- ja kontrolliryhm\u00e4\u00e4n. Liikuntaryhm\u00e4l\u00e4iset noudattivat yksil\u00f6lli-sesti suunniteltua progressiivista kest\u00e4vyysliikuntaohjelmaa. Kontrolliryhm\u00e4l\u00e4iset ohjeistettiin jatkamaan tavanomaista el\u00e4m\u00e4\u00e4ns\u00e4. Itsearvioitua unen laatua ja kestoa, nukahtamisviivett\u00e4, y\u00f6llisten her\u00e4ilyjen m\u00e4\u00e4r\u00e4\u00e4, univaikeuksien yleisyytt\u00e4 ja uneliaisuutta arvioitiin kyselylomak-keiden ja unip\u00e4iv\u00e4kirjojen avulla. Lis\u00e4ksi arvioitiin fyysisen aktiivisuuden m\u00e4\u00e4r\u00e4\u00e4, maksimaa-lista hapenottokyky\u00e4, painoindeksi\u00e4 ja vy\u00f6t\u00e4r\u00f6nymp\u00e4ryst\u00e4.\r\n\r\nIntervention aikana liikunta- ja kontrolliryhm\u00e4n v\u00e4lill\u00e4 ei ollut eroa kohtuutehoisen liikunnan m\u00e4\u00e4r\u00e4ss\u00e4 (p=0.27), mink\u00e4 seurauksena ryhmien v\u00e4list\u00e4 eroa ei saatu aikaan my\u00f6sk\u00e4\u00e4n nukah-tamisviiveeseen (p=0.52), y\u00f6llisten her\u00e4ilyjen m\u00e4\u00e4r\u00e4\u00e4n (p=0.14), unen laatuun (p=0.13), unen kestoon (p=0.20), uneliaisuuteen (p=0.20) tai eri univaikeuksien yleisyyteen (p=0.11\u20130.96). Liikuntaryhm\u00e4 paransi kuitenkin maksimaalista hapenottokyky\u00e4\u00e4n (9.7 %, p=0.11) ja pienensi vy\u00f6t\u00e4r\u00f6nymp\u00e4ryst\u00e4\u00e4n (1.7 %, p=0.04) enemm\u00e4n kuin kontrolliryhm\u00e4.\r\n\r\nKoska kohtuutehoisen liikunnan m\u00e4\u00e4r\u00e4ss\u00e4 ei ollut eroa liikunta- ja kontrolliryhm\u00e4n v\u00e4lill\u00e4, liikuntaryhm\u00e4ss\u00e4 ei saatu aikaan tilastollisesti merkitsev\u00e4\u00e4 parannusta verrattuna kontrolli-ryhm\u00e4\u00e4n. Because the most usual cure of insomnia sedatives increases the risk of many adverse events, it is im-portant to examine other ways to treat insomnia. Objective of this study is to assess the effect of aerobic exercise on sleep quality and duration in middle-aged men reporting insomnia.\r\n\r\nThis was randomized, controlled trial where forty-two sedentary volunteer men from Central Finland health care district aged 30 to 65 years with insomnia (mean age 51 [SD 1.6] years) were allocated to an exercise and a control group. The exercise group participants followed individually tailored progressive aerobic exercise program. The control group participants were asked to maintain their usual daily activities. Self-reported sleep quality, duration and latency, amount of awakenings, frequency of sleeping difficulties and daytime sleepiness were assessed using questionnaires and sleeping diaries. Amount of physical activity, maxi-mal oxygen uptake, body mass index and waist circumference were measured.\r\n\r\nDuring the intervention, both exercise and control had performed similar amount of moderate intensity exercise (p=0.27). Consequently no significant effect on sleep latency (p=0.52), awakenings at night (p=0.14), sleep quality (p=0.13), sleep duration (p=0.20), daytime sleepi-ness (p=0.20) or on frequency of any sleeping difficulties (p=0.11 to 0.96) were found be-tween the exercise and control groups. Despite of this, exercise group had significantly im-proved their maximal oxygen uptake (9.7 %, p=0.01) and reduced their waist circumference (1.7 %, p=0.04) compared to the control group.\r\n\r\nDue to the similar participation of moderate intensity exercise in both exercise and control groups, no significant improvement on quality and duration of sleep were found in exercise group compared the control group. Our results indicated that when subjects have agreed to participating in a lifestyle intervention, they have already committed to change their lifestyle regardless of randomization. Consequently, the effect of exercise could have been shaded by the biased control group.
