Samalla t\u00e4m\u00e4 on lis\u00e4nnyt riski\u00e4 sairastua aineenvaihduntasairauksiin my\u00f6s nuoremmilla. T\u00e4m\u00e4n opinn\u00e4ytety\u00f6n tarkoituksena oli tutkia liikunnan, kunnon ja kehonkoostumuksen yhteytt\u00e4 insuliiniherkkyy-teen 32-37-vuotiailla kaksosmiehill\u00e4.\r\nTutkimukseen valikoitui 23 identtist\u00e4 (MZ) kaksosparia (N=46) aiemmista tutkimuksista saa-tujen liikunta- ja terveystietojen perusteella (suomalainen kaksoskohortti). Kuntotekij\u00e4t m\u00e4\u00e4ri-tettiin polkupy\u00f6r\u00e4ergometritestill\u00e4 (VO2max) ja voimamittaustuolissa (maksimaalinen polven ojennusvoima). Kehonkoostumusmittauksissa k\u00e4ytettiin mittanauhaa, DXA- ja BIA-laitteita. Insuliiniherkkyys (Matsuda) mitattiin 2-h-OGTT:n perusteella, glukoosi- ja insuliiniprofiili m\u00e4\u00e4ritettiin, mitattiin paastotilassa plasman rasva-ainepitoisuudet ja verenpaine. Ty\u00f6matka- ja vapaa-aikaliikunta kartoitettiin kyselyhaastatteluihin perustuvilla MET- ja Baecke sport in-dekseill\u00e4. Indeksien avulla analysoitiin kaksosten (N=46) liikunta-aktiivisuuden yhteydet in-suliiniherkkyyteen, sek\u00e4 onko vaikutus samanlainen liikuntaeron perusteella valituilla 10 kak-sosparilla (n=20) aktiivisia ja inaktiivisia kaksosia verrattaessa. \r\nKaksosilla (N=44) oli normaali (<7.8 mmol/l) sokerinsieto 2-h-OGTT:ss\u00e4. Tulosten mukaan heikentynyt insuliiniherkkyys (Matsuda \u22643.0) todettiin 6.8 %:lla kaksosista. Liikunta-aktiivisuus oli itsen\u00e4isesti yhteydess\u00e4 insuliiniherkkyyteen (42.6 %, p<0.001), mutta ei ollut en\u00e4\u00e4 tilastollisesti merkitsev\u00e4, kun otettiin huomioon muut vaikuttavat tekij\u00e4t lineaarisessa regressiomallissa. Insuliiniherkkyytt\u00e4 selittiv\u00e4t itsen\u00e4isesti VO2max (p<0.001), vy\u00f6t\u00e4r\u00f6pituus-suhde (p=0.011) ja polvenojennusvoima (p=0.022). N\u00e4iden selitysaste oli 66.5 % (p<0.001). Tekij\u00f6iden prosenttiosuudet olivat 41.0 %, 18.5 % ja 7.0 %. Sama tulos n\u00e4kyi my\u00f6s ryhmi\u00e4 vertailtaessa. Ryhmien v\u00e4linen liikuntaero oli merkitsev\u00e4 MET- (p=0.005) sek\u00e4 Baecke sport (p=0.005) indekseill\u00e4. Aktiiviset olivat my\u00f6s paremmassa kunnossa. Heill\u00e4 oli 14.4 % suurempi VO2max (p=0.008) ja 4.7 % polven ojennusvoima. Aktiivisilla oli my\u00f6s 13.8 % pienempi rasvaprosentti (p=0.032), vy\u00f6t\u00e4r\u00f6lantiosuhde 2.2 % (p=0.035), sek\u00e4 plasman triglyseridipi-toisuus 13.3 % pienempi verrattuna inaktiivisiin. Ryhmien v\u00e4lill\u00e4 oli merkitsev\u00e4t erot paas-toinsuliinissa (p=0.047) ja 30 minuutin (p=0.015) insuliiniarvoissa.\r\nTulosten mukaan kuntotekij\u00e4t, VO2max ja polven ojennusvoima, sek\u00e4 vy\u00f6t\u00e4r\u00f6pituussuhde ovat itsen\u00e4isesti yhteydess\u00e4 parempaan insuliiniherkkyyteen, joka on m\u00e4\u00e4ritelty OGTT arvojen perusteella. At the same time they have been shown to increase the risk of metabolic diseases also in younger people. The purpose of this study was to examine how physical activity, fitness and body composition are associated with insulin sensitivity in 32-37-year-old male twins.\r\nThe study involved 23 indentical (MZ) twin pairs identified from a population-based twin cohort. Fitness was measured by maximal oxygen uptake (Vo2max) in a bicycle ergometer test and by maximal force in a chair of knee extension force. Body composition was measured by a tape measure, DXA and BIA (body impedance analyzer). Insulin sensitivity was measured by an oral glucose tolerance test (2-hour OGTT) based Matsuda index, glucose and insulin profile was defined, fasting plasma lipids and blood pressure levels were measured. Commut-ing and leisure-time physical activity was collected with questionnaire and interview and re-ported as MET (metabolic equivalent) and Baecke sport indices. The indices were used to analyze associations with physical activity and insulin sensitivity for all twins (N=46). The difference in insulin sensitivity has also analyzed in a co-twin control design among ten phys-ical activity discordant twin pairs.\r\nThe twins (N=44) had normal glucose tolerance (<7.8 mmol/l) in the 2-hour OGTT-test. Ac-cording to the results there was impaired insulin sensitivity (Matsuda \u22643.0) in 6.8 % of the twins. Physical activity was independently associated with insulin sensitivity (42.6 %, p<0.001), but this was no longer statistically significant after accounting for other elements in the linear regression model. Insulin sensitivity was independently explained by VO2max (p<0.001), waist-to-length ratio (WLR) (p=0.011) and knee extension force (p=0.022). The coefficient of determination was 66.5 % (p<0.001). The explanation rates of these factors were 41.0 %, 18.5 % and 7.0 %. The same result was also reflected in the pair-wise analysis. There were significant differences in physical activity with MET (p=0.005) and Baecke sport (p=0.005) indices. The active twins were also in better fit. They had 14.4 % higher maximal oxygen uptake (p=0.008) and 4.7 % higher knee extension force. Fat percent was also 13.8 % (p=0.032), waist-to-hip ratio (WHR) 2.2 % (p=0.035), and plasma triglycerides 13.3 % lower among the active twins than in their inactive co-twins. There were significant differences in fasting insulin (p=0.047) and 30 minutes (p=0.015) insulin values between groups.\r\nAccording to the results on fitness, VO2max and maximal knee extension force, as well as WLR are independently associated with better insulin sensitivity measured by OGTT. In conclusion, insulin sensitivity is increased when practicing endurance or strength-training regularly or the exercise reduces WLR (WHR).
