Kohdistamalla stimulaatio primaariselle motoriselle aivokuorelle (M1) aiheutetaan lihasvaste (MEP) kehon vastakkaisella puolella. K\u00e4tisyyden vaikutuksesta kortikospinaalisten ratojen toimintaan on esitetty ristiriitaista tietoa. T\u00e4m\u00e4n tutkielman tarkoituksena oli selvitt\u00e4\u00e4 k\u00e4den dominanssin vaikutusta MEP:hin oikea- ja vasenk\u00e4tisill\u00e4. Lis\u00e4ksi k\u00e4tisyyksien v\u00e4lisi\u00e4 eroavaisuuksia tarkasteltiin k\u00e4sien vertailevien toiminnallisten testien avulla.\n\nTutkimukseen osallistui 23 tervett\u00e4 vapaaehtoista henkil\u00f6\u00e4 (22\u201356-vuotta), joista 14 oli oikeak\u00e4tisi\u00e4 ja 9 vasenk\u00e4tisi\u00e4. TMS kohdennettiin k\u00e4den edustusalueelle M1:sen p\u00e4\u00e4lle kansainv\u00e4lisen 10-20-j\u00e4rjestelm\u00e4n mukaan. MEP:t rekister\u00f6itiin EMG- pintaelektrodien avulla k\u00e4den thenar ja hypothenar lihaksista stimuluksen vastakkaiselta puolelta sek\u00e4 levossa ett\u00e4 aktiivisuuden aikana. K\u00e4sien toiminnallisiin testeihin kuuluivat sormien naputusnopeuden ja puristusvoimien mittaukset. Dominantin ja non-dominantin k\u00e4den v\u00e4lill\u00e4 ei havaittu eroavaisuuksia motorisissa kynnyksiss\u00e4 (MT), MEP:ien amplitudeissa, latensseissa tai MEP:n j\u00e4lkeisess\u00e4 inhibitioajassa (SP). Oikea- ja vasenk\u00e4tisten v\u00e4lill\u00e4 havaittiin eroavaisuuksia aktiivisuuden aikana tehdyiss\u00e4 mittauksissa. Oikeak\u00e4tisill\u00e4 oli tilastollisesti merkitsev\u00e4sti matalampi MT oikeassa k\u00e4dess\u00e4 kuin vasenk\u00e4tisill\u00e4. Vasemman k\u00e4den thenarissa oli merkitsev\u00e4sti suuremmat MEP:ien amplitudit vasenk\u00e4tisill\u00e4. MEP:ien amplitudit olivat merkitsev\u00e4sti suurempia sek\u00e4 MEP:ien latenssit ja MT:t olivat merkitsev\u00e4sti pienempi\u00e4 aktiivisuuden aikana kuin levossa. Puristusvoimat ja sormien naputusnopeus olivat merkitsev\u00e4sti suurempia dominantissa kuin non-dominantissa k\u00e4dess\u00e4. \n\nKliinisess\u00e4 k\u00e4yt\u00f6ss\u00e4 on t\u00e4rke\u00e4 tieto, ett\u00e4 dominantin ja non-dominantin k\u00e4den v\u00e4lill\u00e4 ei esiinny merkitsevi\u00e4 eroavaisuuksia MEP:ss\u00e4. Tietoa voidaan hy\u00f6dynt\u00e4\u00e4 erilaisten neurologisten sairauksien ja esimerkiksi kuntoutusennusteen arvioinnissa. Lis\u00e4ksi tulisi kiinnitt\u00e4\u00e4 huomiota, ett\u00e4 vasen- ja oikeak\u00e4tisill\u00e4 henkil\u00f6ill\u00e4 saattaa ilmet\u00e4 erilaisia tuloksia. By placing the stimulation over the primary motor cortex (M1) motor evoked potentials (MEP) can be elicited on the peripheral muscles on the contralateral side of the body. The influence of handedness on the function of corticospinal tract still remains unsolved. The aim of this study was to determine if MEPs are influenced by hand dominance in right- and left-handed people. The differences in handedness were also compared with behavioral functional tests of the hands. \n\nTwenty three healthy volunteers (aged 22-56 years, 14 right-handed and 9 left-handed) participated in this study. TMS coil was positioned on the hand representation area over the M1 according to the international 10-20-system. MEPs were recorded by surface EMG electrodes both at rest and during activation from hand thenar and hypothenar muscles. Behavioral functional tests of the hands were tested by finger tapping tests and grip strength measurements. No difference was found between dominant and non-dominant hand in motor thresholds (MT), MEP amplitudes, latencies or silent periods (SP). There were few differences between right-handers and left-handers during the hand activation. Right-handers had lower MT in right hand than left-handers. MEP amplitudes were higher in the left thenar with left-handers. MEP amplitudes were significantly higher and MTs and MEP latencies were significantly smaller during the activation than in resting measurements. Grip strength and finger tapping rates were significantly higher in the dominant hand.\n\nIn clinical use it is important to notice that there are no differences between dominant and non-dominant hand in MEPs. It is useful to consider this information when evaluating neurological diseases and for example in rehabilitation prediction. Also it ought to be taken into account that left-handers and right-handers might have different kinds of results.
